Jassim البكتريولوجي زينب حميد غني*، أ.د.ميعاد كاظم حسن** و أ.د.عبد المحسن حميد جاسم*** PY - 2008 VL - 26 IS - 2 SP - 121 EP - 126 JO - The Medical Journal of Basrah University المجلة الطبية لجامعة البصرة SN - 02530759 AB - ABSTRACTObjectives: A prospective study was carried out to study the outcome of patients with visceral leishmaniasis and todetermine factors associated with poor outcome.Methods: The study included 132 in-patient children who were admitted to Basrah Maternity and Children Hospitaland Basrah General Hospital during one year(from the first of November 2004 till the end of October 2005), withvisceral leishmaniasis confirmed by bone marrow examination and direct agglutination test.Nutritional assessment was done for each patient. All patients with confirmed visceral leishmaniasis were sent forcomplete blood count. The final diagnosis and the outcome of the patients with visceral leishmaniasis were recordedalso.Results: Among 132 sero-positive cases, 78 (59.0%) cases were males, and 54(40.9%) were females. The age ofpatients ranged between 2 months to 12 years.Sixty five (49.2%) of the cases were improved, 31.8% were discharged on the family responsibility, and 15 cases died.The commonest cause of death was bleeding in 6 cases (40%), followed by hepatic failure in 4 (26.6%), other causesof death were bronchopneumonia, renal and heart failure.Nutritional status and duration of illness were significant determinants of the outcome of visceral leishmaniasispatients. A significantly higher number of malnourished patients didn’t improve compared to well nourished patients,and a significantly higher percent of patients who presented late have died compared to those who presented earlierespecially in the first 2 weeks of illness, P<0.05. By using multivariate regression analysis, six variables; lowhemoglobin value, low platelet count, male sex, young age, high Direct Agglutination Test titer and malnutritionwere found to be significant predictors of death and relapse.Conclusion: Male sex, anemia, thrombocytopenia, high DAT titer and malnutrition are poor prognostic factors inaddition late presentation.

ماجستير أحياء مجهرية/مستشفى الصدر التعليمي/البصرة، **فرع طب الأطفال/ كلية الطب/جامعة البصرة و ***فرع الأحياء المهجرية/كلية الطب/ جامعة البصرةالهدف:أجريت هذه الدراسة المستقبلية لمعرفة مصير المرضى المصابين التي باللشمانيا الحشوية والعوامل التي قد تؤثر سلبا على مصير المرضى. الطريقة:شملت الدراسة 132 طفلا تم إدخالهم إلى مستشفى البصرة للنسائية والأطفال ومستشفى البصرة العام خلال عام واحد (للفترة من تشرين الثاني 2004 ولغاية تشرين الثاني 2005) والذين تم تشخيصهم بواسطة اختبار التلازن المباشر وفحص نخاع العظم. تم تقييم الحالة التغذوية لجميع المرضى، كما إرسال نموذج دم لدراسة نسبة خضاب الدم، الكريات الدم البيضاء والأقراص الدموية. تم متابعة المرضى خلال فترة الرقود وتسجيل مصير كل مريض.النتائج: من بين 132مريضا تم تشخيصهم بواسطة اختبار التلازن المباشر، 78 كانوا من الذكور (59%) و 54 (40.9%)من الإناث. أعمار المرضى تراوحت بين شهرين الى 12 عاما، 74.3% كانت أعمارهم اقــل مـن سنتين, خمسة وستون حالـة قد اكتسبوا الشفاء التام (49.2%)، بينما ( 31.8%) قد أخرجوا من المستشفى على مسؤولية الأبوين وبدون معرفة مصيرهم، وقد تم تسجيل 15 حالة وفاة، كانت أكثر أسباب الوفيات هي النزف الدموي 6 حالات (40%) ثم الفشل الكبدي في 4 حالات (26.6%) ويليها ذات الرئة (20%) والفشل الكلوي وفشل القلب (6.6%) لكــل منهما أثبتت الدراسة ان الحالة التغذوية للأطفال المصــابين وفترة المرض من العوامل المؤثرة على حصيلة المرض وكان الفرق معتد إحصائيا (0.05>). أظهرت الدراسة ان هناك 6 متغيرات لها فارقا معنويا مع تنبؤات الوفاة والانتكاس وهي (قلة نسبة الهيموكلوبين، قلة عدد الأقراص الدموية، جنس الذكور، الأعمار الصغيرة، سوء التغذية والمعيار العالي لاختبار التلازن المباشر. الاستنتاج: جنس المريض (الذكور)، فقر الدم، نقص الأقراص الدموية، المعيار العالي في اختبار التلازن، الأعمار الصغيرة وسوء التغذية هي عوامل مرتبطة بالوفاة إضافة الى التشخيص المتأخر.المجلة الطبية لجامعة البصرة ص121-126 ER -
