Renal injuries are the most common injuries of the urinary system. A blunt trauma directed to the abdomen,flank, or back is the most common mechanism. A kidney with a pre-existing abnormality is at increased risk forinjury. The objective of the study is early detection of intraperitoneal rupture of the kidney in cases of acute abdomenand also to confirm the fact that the severity of patients' symptoms is disproportionate to the degree of injury inchildren suffering from renal abnormalities.Patients & methods: A total of 15 patients were included in the study from 2004-2009. The age range was 3-13years. Five patients were girls & ten were boys. A brief history was taken and a thorough physical examination wasdone. Hemoglobin, packed cell volume and urinalysis were done. Imaging by ultrasonography and computerizedtomography (CT) with contrast were done in these patients. Emergency laparotomy was performed in all patients.Results: Intraperitoneal rupture of the kidney with urine peritonitis (uroperitoneum) was found in all patients.Seven patients had left sided renal injuries while eight patients had right sided injuries. A thin renal parenchyma withextravasation of contrast media into the peritoneal cavity were found in CT of all patients. Nephrectomy was thedecided surgical treatment in every patient.Conclusion: It can be concluded that children with hydronephrotic kidneys are more prone to intra-peritonealrupture from minor trauma and a high index of suspicion is needed to diagnose those patients with intraperitonealrupture of the kidney.
فرع الجراحة/كلية الطب/جامعة البصرة/العراقمقدمة:حوالي10% من الإصابات الموجودة في غرف الطوارئ في المستشفيات تشمل إصابات الجهاز البولي والتناسلي. تعتبر إصابات الكلى من أكثر الإصابات شيوعا" في الجهاز البولي وقد تحدث نتيجة ضربات مباشرة على البطن أو منطقة الظهر إن الكلية المريضة تكون معرضة لخطر الإصابة أكثر من غيرها. إن الهدف من الدراسة هو إلقاء النظر والتشخيص المبكر لحالات البطن الحادة الناتجة عن التمزق الحاصل في الكلية المريضة نتيجة شدة خارجية بسيطة على البطن.المرضى وطريقة العمل: خمسة عشر مريضا" أشركوا في الدراسة منذ عام 2004 ولغاية 2009 وكانت أعمارهم تتراوح بين 3-13 سنة وقد كان عدد الإناث (5) وعدد الذكور (10). أجريت الفحوصات السريرية والمختبرية والشعاعية لجميع المرضى كما أجريت عمليات فتح بطن استكشافية لهم.النتائج: وجود تجمع بولي في الصفاق ناتج عن تمزق في الكلى اليسرى (7 مرضى) وفي الكلى اليمنى ( 8 مرضى) مع وجود توسعات شديدة في الكلى المصابة. أجريت عملية استئصال الكلى المصابة لجميع المرضى.الاستنتاج: يمكن الاستنتاج بأن الأطفال الذين يعانون من أمراض مسبقة وتوسعات شديدة في الكلى يكونون أكثر تعرضا" لحالات البطن الحادة الناتجة عن شدة خارجية بسيطة على البطن وتمزق الكلى المصابة. المجلة الطبية لجامعة البصرة ص33-35} }
