PY - 2009 VL - 12 IS - 2 SP - 306 EP - 324 JO - KUFA MEDICAL JOURNAL مجلة الكوفة الطبية SN - 1993517X 27094462 AB - This study was carried out on 20 normal apparently healthy male volunteers, aged 18-36 years in order to study the effect of deep inspiration and forced expiration on mitral valve inflow,cardiac output, by using non-invasive techniques. Mercury sphygmomanometer for measuring the blood pressure (BP), Doppler echocardiography for estimating of stroke volume (SV), cardiac output (CO), velocity time integral (VTI), Mitral valve area (MVA), pressure half time (PHT), acceleration, deceleration time of passive atrial filling E wave, acceleration time(T1), deceleration time (T2) and acceleration time (T3),deceleration time (T4) of atrial contraction, A wave. The results: SV,CO,VTI were significantly higher and PHT significantly lower in baseline expiratory than inspiration. There were no significant changes in MBP,HR,T1,T2,T3 and T4. Comparison between deep inspiration and forced expiration shows that the MBP, SV, CO, VTI, PHT and T1were significantly higher during forced expiration than that of deep inspiration, where as there are no significant differences in HR, T2, T3 and T4. Comparison between baseline quite breathing (inspiratory phase) and deep inspiration:The results indicate that there were no significant changes in SV,CO, MBP,HR,VTI,PHT,T1,T2,T3 and T4 between baseline and deep inspiration.Comparison between control and forced expiration, shows no significant changes in SV, CO, HR, MBP, VTI, PHT, T1,T2, T3, and T4.The aim of the present research is first: to investigate the changes of mitral valve inflow and its effect on cardiac output during deep inspiration and forced expiration.

أجريت هذه الدراسة على ٢٠ متطوع من الذكور الأصحاء والذين تتراوح أعمارهم من ١٨-٣٦ سنه وذلك بهدف دراسة انعكاسات القلب والأوعية الدموية استجابة للشهيق والزفير الاعتيادي والشهيق العميق والزفير القهري، بواسطة استخدام جهاز الضغط الزئبقي لقياس ضغط الدم وجهاز الايكو الدوبلر لقياس نبضات القلب(HR)،كمية الدم التي يضخها القلب في النبضة الواحدة (SV)، كمية الدم التي يضخها القلب في الدقيقة الواحدة (CO)، سرعة الوقت المتكاملة (VTI)، مساحة الصمام الأكليلي (VAM)، ضغط نصف الوقت(PHT)، وقت التصاعد ووقت التباطؤ لموجة E (الامتلاء الكامن ) (1T،2T) ،ووقت التصاعد والتباطؤ لموجة A(انقباض الأذين) (4T، 3T).وقد تبين من التحليل الإحصائي انه هناك زيادة معنوية في مقدار ما يضخه القلب في النبضة الواحدة وما يضخه في الدقيقة الواحدة وسرعة الوقت المتكاملة و تناقص في ضغط نصف الوقت في الزفير الاعتيادي مقارنة بالشهيق،كذلك لا يوجد تغيير في معدل ضغط الدم ،نبضات القلب ،الوقت التصاعدي والتنازلي لكلا الموجتين،EوA.وبمقارنة المتغيرات في الشهيق العميق بالزفير القهري وجد ان هناك زيادة ملحوظة في معدل ضغط الدم،معدل ضخ القلب في النبضة الواحدة وفي الدقيقة الواحدة وسرعة الوقت المتكاملة وضغط نصف الوقت ومقدار الوقت التصاعدي للموجة E في الزفير القهري اكبر من الشهيق العميق ،في حين التغير في نبضات القلب ،وقت التباطؤ لموجةE ووقتي التسارع والتباطؤ لموجةA هو غير ملحوظ.وبمقارنة التغيرات في الشهيق والزفير الاعتيادي مع كل من الشهيق العميق والزفير القهري لوحظ عدم وجود تغير ملحوظ وفي جميع المتغيرات.
