Thejeel رباب ثجيل فرحان PY - 2014 VL - 56 IS - 1 SP - 53 EP - 56 JO - Journal of the Faculty of Medicine Baghdad مجلة كلية الطب SN - 00419419 24108057 AB - Back ground: Celiac disease (CD) is an immune–mediated enteropathy induced by gluten with broad spectrum of presentation, many patients with atypical symptoms or clinically silent remain undiagnosed, and are at risk of long-term complications.Objective: The aim of the present study is to determine the of prevalence celiac disease in children with short stature.Patient and Methods: In this prospective study, 307 children 181(58.9%)males and 126(41.1%) females ,with ages ranging from 3 to 17.5years ( mean 12.2 ±3.4) with short statures (height below 3rd percentile adjusted for age and sex) attending the consultation room of endocrinology/ Welfare Teaching Hospital/medical city- Baghdad , were enrolled in this study from the first of Oct 2008 to the first of Aug 2010.Full history, thorough physical examination, immune-biochemical tests ; complete blood count ,renal function tests, serum electrolytes, thyroid function tests, immunoglobulin A and G, tissue transglutaminase (tTG) antibodies , and intestinal biopsy for those with positive celiac serological screens, growth hormone estimation, radiological assessment of bone age, genetic counseling for some females (when available) was done . Each celiac disease case once diagnosed was started on gluten free diet and followed up for one year for growth parameters.Results: celiac disease (CD) was found as the second most common cause of short stature. Unfortunately there was delay in the diagnosis due to asymptomatic status of the disease. Complete catch up in growth had been found in 63.6% after one year of gluten free diet.Conclusion: Celiac disease is an important cause of short stature in children, and should be an essential part of screening panel and workup of short children, irrespective to the presence or absence of gastrointestinal symptoms.Key words: Celiac disease, short stature, anti tissue transglutaminase antibody, children, gluten free diet.

خلفية: مرض حساسية الحنطة هو من الامراض المتوسطة مناعيا معويا و الناجمة عن الحث بواسطة الغلوتين مع طيف واسع من العرض، العديد من المرضى الذين يعانون من أعراض غير نمطية أو صامتة سريريا تظل دون تشخيص، ومعرضون لخطر حدوث مضاعفات على المدى الطويل.الهدف: الهدف من هذه الدراسة هو تحديد نسبة انتشار المرض الزلاقي في الأطفال الذين يعانون قصر القامة.المرضى و طرق العمل : اجريت هذه الدراسة المستقبلية ، على 307 أطفال 181 (58.9٪) ذكور و 126 (41٪) إناث، مع الأعمار التي تتراوح بين 3 إلى 17.5سنة (متوسط 12.2 ± 3.4) الذين يعانون من القامات القصيرة (الطول قل من النسبة 3 المقترحة للعمر و الجنس) يحضرون العيادة الاستشارية الغدد الصماء / المستشفى التعليمي الرعاية الطبية / مدينة بغداد، في هذه الدراسة من أول أكتوبر 2008 إلى الأول من أغسطس 2010. التاريخ العائلي ، فحص شامل على الجسم، واختبارات المناعة والكيمياء الحيوية؛ الدم الكامل. اختبارات وظائف الكلى، والشوارد في الدم، واختبارات وظائف الغدة الدرقية، الغلوبولين المناعي A و G، الغلوتامين الأنسجة (TTG) الأجسام المضادة، و تحري خزعة الأمعاء لذوي الاضطرابات الهضمية المصلية الإيجابية، وتقدير هرمون النمو، وتقييم الإشعاعية في سن العظام، وتقديم المشورة الوراثية لبعض الإناث ( عندما تكون متاحة وذلك). بدأ كل حالة تشخيص الداء الزلاقي مرة واحدة على نظام غذائي خال من الغلوتين ومتابعتها لمدة سنة واحدة لمعلمات النمو.النتائج: تم تسجيل ان مرض الاضطرابات الهضمية (CD) كثاني سبب الأكثر شيوعا من اسباب قصر القامة. للأسف كان هناك تأخير في التشخيص بسبب عدم توضح أعراض المرض. تم تسجيل تحسن النمو في 63.6٪ لدى المرضى بعد سنة واحدة من حمية الغلوتينالاستنتاج مرض السيلياك هو أحد أهم أسباب قصر القامة عند الأطفال، ويجب أن تكون جزءا أساسيا من التحري و التطبيقي في الأطفال قصار القامة ، بغض النظر عن وجود أو عدم وجود أعراض الجهاز الهضمي.مفاتيح الكلمات: مرض السيلياك، قصر القامة، الأجسام المضادة للغلوتامين في الأنسجة ، حمية الاطفال خالية من الغلوتين ER -
