Ki 67 Expression in Various Diseases of Gallbladder

Abstract

Cell proliferation is an important prerequisite for the development of a neoplasm. Ki-67 is one of the most important cell proliferation markers. Its expression is correlated with the aggression of various histopathologic changes in the epithelium of gallbladder. This study aims to assess the expression of Ki-67 in various pathological processes that affects the epithelium of gallbladder.Gallbladders from 146 patients were studied histopathologically. Then sections from the paraffin embedded tissues were evaluated for the expression of Ki-67 by using immunohistochemical method. The results of Ki-67 labeling index were estimated for each pathologic category and were analyzed for their significance using Mann-Whitney test.The mean value of Ki-67 LI in the epithelium of normal gallbladder was (0.7%), in acute cholecystitis (1.14%), epithelial hyperplasia (8.38%), metaplasia (21.18%), dysplasia (38%) and (70.6%) in the carcinoma group. There was no statistically significant difference with regard to ki-67 between the epithelium of normal GB and those with acute inflammation (p= 0.904) and the difference was highly significant when the normal gallbladder epithelium was compared with carcinoma group. There was statistically highly significant difference when the comparison was performed between normal epithelium and metaplasia group (p< 0.001) and the group of dysplasia (p= 0.001).From the present study a conclusion can be made is that Ki-67 expression increases in chronic inflammation and further increment occurs in premalignant and malignant lesions of the gall bladder epithelium and can be used as a marker of aggression of histopatholgic lesions.

The correlation of stromal CD 10 expression in Invasive breast cancer with various clinico-pathological parameters

Abstract

AbstractBackground: CD10 is a cell surface zinc-dependent enodpepitdase and its expression in tumor stroma is thought to be associated with biological aggressiveness of many epithelial malignancies. Objectives: To estimate the frequency of stromal CD10 expression in invasive breast cancer and its correlation to clinico-pathological data including immunohistochemical markers.Methods: This study included 102 cases of invasive breast carcinoma. CD10 expression was assessed by immunohistochemistry and scored as negative, weak and strong positive and the results were statistically analyzed in correlation to various clinical and pathological parameters in addition to other immunohistochemical markers.Results: CD 10 was found to be positive in 58/102 cases (56.8%). Stromal CD10 showed significant correlation with the T parameter of the TNM staging system (p=0.043), dermal infiltration (p=0.049) and estrogen receptor positivity (p=0.039), while there was no significant correlation with the patients’ age (p=0.20), tumor grade (p=0.294), and lymph node status (p=0.29).Conclusion: Stromal CD10 was expressed in more than half of cases with invasive breast carcinoma, and there was a significant correlation of the stromal CD 10 expression with the T parameter of the TNM staging system, dermal infiltration and the positivity of estrogen receptor expression and no correlation was found between stromal CD10 expression and patients’ age, tumor grade, stage, lymph node status, lymphovascular invasion, perineural invasion, Paget’s disease of the nipple, progesterone receptors expression and HER2/Neu status.

الهدف من الدراسة: هو رصد ظهور CD10 في سدى انسجة سرطان الثدي عن طريق الفحوصات المناعية , و تقيم علاقته مع المعلومات السريرية و المرضية مثل العمر، حجم الورم، درجة الورم، نوع الورم، مرحلة الورم، غزو الأوعية اللمفاوية والدموية، وضع العقدة الليمفاوية والفحوصات المناعية الاخرى.المواد والطرق: شملت هذه الدراسة على 102 عينة لسرطان الثدي مثبتة بالفورمالين ومطمورة بالشمع وقد تم اخذ العينات من ارشيف المختبر المركزي للصحة العامة في دهوك للفترة الممتدة من 2010 ولغاية2016. تم رصد تفاعل خلايا السدى مع 10 CD وسجلت بانها سلبيه, ضعيفة وقوية ايجابيا وتم تحليل هذه النتائج احصائيا لاظهار علاقة بين تعبير CD10 مع مختلف المعايير السريرية و المرضية بالاضافة لفحوصات المناعية الاخرى. النتائج: كان تفاعل ال CD10 ايجابيا مع ٥٨ حالة من اصل ١٠٢حالة وقد اظهرت النتائج ارتباط احصائي بين ظهور ال CD10 في سدى سرطان الثدي مع المعلمه Tمن نظام ال TNM لتصنيف الاورام0.043%) (p value=، وتسلل عن طريق الجلد (p value=0.049) و مستقبلات هرمون الاستروجين الايجابية (p value=0.039). في حين لم يكن هناك ارتباط احصائي مع عمر المرضى (p value=0.20)، ودرجه الورم(p value=0.294)، و وضع العقد اللمفاوية (p value=0.29).الاستنتاج: ظهر تفاعل ال CD10 مع سدى سرطان الثدي في اكثر من نصف الحالات المدروسة، وكان هناك ارتباط احصائي مع المعلمة T من أل TNM لتصنيف الاورام، و التسلل عبر الجلد ومستقبلات هرمون الاستروجين الايجابية، ولم يتم العثور على علاقة احصائية مع عمر المرضى، درجة الورم، وتصنيف الورم، وضع العقد اللمفاوية، غزو الاوعية الدموية واللمفاوية، غزو حول العصب، مرض باجيت للحلمة، ومستقبلات الاستروجين والبروجيستيرون ووضع .HER2/Neu

Abstract

Background and objective: The presence of bile-resistant Helicobacter spp. in bile and gallbladder tissues has been proposed as a cause of gallbladder disease. This study aimed to assess the histopathological events in calculus cholecystitis and investigate the presence of the DNA of Helicobacter spp. in the bile. Methods: Forty patients who underwent cholecystectomy for calculus cholecystitis were evaluated for the presence of Helicobacter spp. by polymerase chain reaction methods and gallbladder tissue was evaluated histopathologically. Gall bladders were fixed in formalin and subjected to histopathological investigation while bile sample was used for the extraction of DNA.Results: The histopathological findings revealed that twenty-seven (67.5%) samples showed mild chronic inflammation, one (2.5 %) sample with acute inflammation, four (10 %) samples with acute on chronic inflammation, eight (20 %) samples have normal histological findings. The molecular investigation revealed that twenty one (52 %) samples out of the forty showed the presence of genomic DNA after being subjected to DNA extraction and that fourteen (66.7 %) samples out of the twenty one showed amplification bands of 300 bp after PCR amplification indicating the presence of Helicobacter spp. within the samples. But no sample revealed the presence of H. pylori DNA when PCR was applied. Conclusion: Helicobacter spp. are considered as a determining factor for the development of gall bladder inflammation.

Gastric Malignancies and the Trend of Gastric Carcinoma in Duhok City-Iraq

Abstract

Gastric cancer is one of the common human malignancies particularly gastric adenocarcinoma. The stomach also represents the commonest gastrointestinal site for extra-nodal non-Hodgkin’s lymphoma. This study aims to find out the major types of gastric malignancies in Duhok city, their age and gender distribution and the trend of the types of gastric adenocarcinoma over a six year period from January 2008 till December 2013. This study included nearly all cases of gastric malignancies diagnosed in Duhok City and enrolled 155 cases. The type of malignancy was diagnosed by histopathological examination and supported in some cases by immunohistochemical staining procedures and investigations. The trend of the two major types of gastric carcinoma was determined over this six year period. The results of this study showed that the carcinoma constitutes the largest proportion of gastric malignancies (87.7%), followed by lymphoma (6.5%), then Gastrointestinal stromal tumor (GIST) (4.5%) and lastly neuroendocrine tumors (1.3%). The mean age of the intestinal type (62.1 years) was higher than that of the diffuse type (50.15 years) and the difference was statistically significant (P: < 0.05). Similarly the diffuse type was more common in women than men and also the difference was statistically significant (P: < 0.05). The results of this study show that the trend of intestinal type over a six year period is on the rise side while the trend of the diffuse type is more or less the same. Gastric carcinoma is the most common gastric malignancy in Duhok City with a male predominance and most patients had some bad prognostic factors and there is no decline in the prevalence of the intestinal type.
