Abstract

Background: Patients who undergo craniectomy or removal of part of the skull bone following trauma to give a release to the brain are later managed by auto graft, or synthetic acrylic graft.
Patients and method: 20 patients transferred to specialized surgical hospital from U.S mellitary Ibn Sinna hospital with removal the skull bone implanted in the abdomenol wall or thigh and managed by returning of the bone to the scalp compared with 20 patients managed in our hospital to start by craniectomy and implantation of a synthetic bone graft acrylic, the comparism included the surgery, time of delay, out come, complication.
Results: We divided the patients to group A and B, A were the ones treated by Autograft & B by synthetic graft (Acrylic). In both groups half of the patients were between 20 and 40 years of age, with male predominance, the delay of surgery were more group B, but the complications were more in group A, & 25% required surgical intervention & removal of the graft.
Conclusion: we think that Acrylic cranioplasty is better than Auto graft cranioplasty in traumatic cases as it carries better cosmetic results & final out come.

Abstract

Background: dorsal spine intervertebral disc prolapse (IVDP) not a very common entity compared with cervical & lumbar region usually treated surgically.
Patients & method: 22 patients studied in the specialized surgical hospital neurosurgical department from Jan 2002 till Jan. 2006. the study included age, gender, cases. clinical features, diagnoses & surgical management.
Results: 22 patients were studied 76% of the patients are at the age of 30-60 with slight male predominance, all diagnosed by MRI & or CT scan, all managed surgically by laminectomy the results are compared with other studies.
Conclusion: posterior thoracic laminectomy at the dorsal region is a safe, simple procedure with good results if done early & meticolous.

Abstract

CT scan features of Malignant Meningioma
Objective: Meningioma which is a common benign Brain tumor can be malignant, but the final decision is by biopsy. However CT. Scan features can help to evaluate malignancy of a lesion. The aim of this retrospective study is to look for the malignant features of a malignant meningioma .
Patients and Methods: This is a retrospective study of 50 malignant meningiomas in the neuro-surgical unit in the specialized surgical hospital (previously AL-Shaheed Adnan hospital), for the period from March 1992 to March 2007. We search specially for Malignant features on the CT scan study of the patients.
Results: 94%of the patients had different features of malignant meningioma, only 6% CT scan did not show any malignant features, we also evaluated the results of surgery .
Conclusion: CT scan is one of the most important diagnostic procedures for diagnosis of brain tumor; however final diagnosis can only be emphasized by the histo-pathological studies.

Abstract

Background: the management of gun shoot wounds of the spinal cords (G sws) is still a
debate whether to interfere surgically or not, the interferance is usually laminectomy & Bullet
extraction.
Objectives: A comparative prospective study between surgical & non-surgical management of
G sws.
Methods: A case series study of (52) patients with G sws were managed by the neurosurgical
unit in Ibn Sinna hospital in Mosul. 27 patients surgery was done, 25 patients non-surgical
management.
A comparative study done according to the surgical outcome, improvement, mortality between
the 2 groups in a mean follow up period of 4.5 months.
Results: The mean age was 35, (9-50 years) , mean follow up 4.5 months (3-7 months)no
patients with non-surgical treatment show any improvement while 10 out 13 of incomplete
injury show significant improvement by surgery, the mortality rate was also higher in the non-
surgical group.
Conclusion:Bullet in the spinal cord with partial injury of the cord should be removed with
decompression of the cord complete injury of the cord show poor prognosis with or without
surgical intervention.

Abstract

Background: Brain abscess is collection of pus in the brain parrynchima surrounded by a true capsule. Usually diagnosed by CT & MRI, & treated surgically by drainage by burr hole, or excision.Objective: evaluate our work with brain abscess.Patient& method: 74 Patients collected in the specialized surgical hospital neuro-surgical department, from Jan. 1995 till Jan. 2005 treated surgically, all cases fully evaluated clinically & radiologically & then evaluation of the surgical procedure.Results: there is a slight male predominance & prevalence more in the 1st 2decades of life mostly in children with cong. heart disease, headache was the most common presenting feature, with other signs of infection diagnosis was mostly by CT scan, all cases were managed surgically & the out come is compared other studies.Conclusion: Brain abscess a relatively common disease, each case should be managed individually & depending on surgeon experience.

Abstract

Background: carpal tunnel syndrome (CTS) a relatively common disease affecting ♀ morethan ♂, usually at the age between 40 -60 , diagnosed by EMG & NCS, treated when moderateto severe by surgical decompression of the median nerve.Objective: To study the role of surgery in the management of CTS.Patients & method: 124 patients collected from the neuro-surgical departments of thespecialized surgical hospital from August 2001 till March 2007, all patients studied thoroughlyregarding age, sex, associated diseases, clinical features, EMG studies, surgery & out come &follow up for at least one year.Results & discussion: there was around 2:1 ♀:♂ , with age predomena between 40-60 years.All the patients had pain (aching) & most of them numbness, & tingling and only 1/3 hadatrophy of muscle of the hand all diagnosed by EMG & NCS, treated surgically results weregood compared with other studies regarding complications & final out come.Conclusion: surgery for CTS is safe & successful surgery under meticulus procedure &appropriate choice of patients.Keywords: Carpal tunnel syndrome (CTS), Median n. surgical decompression

Abstract

Objectives: a study of surgical procedures used in the management of sciatic n injury & acomparism between them including comparism with other universal studies.Patients and methods: 62 patients collected from the neuro-surgical unit in the specialized surgical hospital from Jan-2004 till October 2007, all patients studied thoroughly regarding age, Gender, type of injury, time of injury & outcome.Results: 85% of our patients were پ‰, 45% were in the age of 21-30 years, most injured by bullet or shells, the delay in surgery was mostly 1-3m, the repair is by direct suture 45%, 29% release of adhesions 16%, excision of neuroma & suturing, 9.6% by nerve graft, the direct suturing carried the best results, & grafting the worse type, comparism done with universal studies.Conclusion: Direct suturing of neural sheath (Epineurium) caries the best results if done at proper time.

Abstract

Background: penetrating foreign body is a common problem but penetration by screw driver is very uncommonPatient & method: A nine years old boy presented with penetrating screw driver through the medial canthus of the eyeResult: the foreign body removed under GA with good result.Conclusion: minimal invasive procedure are preferred when we have penetrating head injury.

Abstract

Background: cerebellar astrocytroma is the commonest paediatric posterior fossa tumor.Patients and method: 54 patients taken from the neuro-surgical unit in the specialized surgical hospital from Aug 1995 till March 2003, all patients studied thoroughly age, gender, Presentation, Radiological diagnoses, surgery, additive therapy & final outcome including 5 years survival & conclusion is reached.Results and discussion: the results show that more than half of the patients were at age 5-7 years with very slight male predominance, most cases diagnosed by CT, some with MRI or MRI alone.Surgery was done for all cases & 16 patients' preceeded by VP shunt. The final out come is compared with other similar studies.Conclusion: cerebellar astrocytoma acommon post fossa pediatric tumor if diagnosed early & proper surgery is done early, good prognosis can be gained.

Abstract

Summary:Background: Gliomas are brain tumours of supporting tissue of the brain. The management of low grade glioma is still a great debate ranges from just follow up to extensive surgery followed by DXT, with or without chemothery.Patients and Methods: 282 patients taken from neuro-surgical hospital between 1980-1990, divided to 3 groups with different modality of management & follow up for 15 years. Results: the five years survival and symptomatic improvement was higher in the group managed by extensive surgery and DXT with or without cytoxic drugs followed by group managed by biopsy and DXT with or without cytoxic.Conclusion: management of low grade glioma symptoms & out come (survival) is best by extensive surgery & DXT with or with out cytotoxic, followed by biopsy & DXT with or without cytotoxic drugs.Keywords: glioma, DXT, cytotoxic drugs.
