Abstract

Background:The reconstruction of facial deformity has been of paramount clinical concern for many years and one of the most difficult and challenging tasks facing the maxillofacial surgeon. The ultimate goal in the treatment is the relief of suffering, restoration of function of jaw, restoration of speech, regain of the normal looking contour and improvement in the quality of life.Objective:To obtain more knowledge on autogenous on lay bone graft behavior in different facial defects and to evaluate the lateral and medial surgical approaches to the iliac crest.Methods:A prospective study was conducted during the period from January 2009 to January 2012 on 20 patients with facial defects, in the orbit, zygoma, maxilla and mandible. The causes of defects were trauma, odontogenic tumors and alveolar cleft. Types of bone graft used were either block cortico-cancellous or chips cancellous bone, the block was either monocortical or bicortical bone graft.Results:Complications associated with donor site harvesting procedure included pain 5% and gait disturbance 5%. Failure of bone graft was observed in 3 patients (15%) while the rest, 17 patients, (85%) ended with functional and esthetic successful graft. Causes of failure were due to sequestration (5%), inflammation due to osteomesh (5%) and soft tissue breakdown (5%).Conclusion:Iliac crest graft has evolved as a safe, well accepted procedure, with relatively low morbidity that can be used for a wide variety of maxillofacial procedures.Keywords:Iliac crest, bone graft, facial defects

Abstract

BONE SPUR DRILLING, A NEW TECHNIQUE TO CORRECT DEVIATED MAXILLARY CREST Sajjad Halboos Mohammed Almansoori MB, ChB, FICMS, CABS ENT. Consultant ENT Surgeon, Alsadr Teaching Hospital, Basrah, IRAQ. Abstract The nasal septum consists of both bony and cartilaginous components which could be deviated resulting in different types of deformities that lead to unilateral or bilateral nasal obstruction in addition to dryness, crustation, and bleeding from the nose. Headache and facial pain may be other presenting features and sinusitis is a possible complication. ENT surgeons tried different surgical techniques to correct these types of septal deviations to ensure the best results and least complications. Forty eight patients (32 males and 16 females) were admitted to Al-Sadr Teaching Hospital with septal deviation in the form of bony spur alone or associated with other nasal septal deflections. Correction of the deviated crest was performed following good exposure by elevating mucoperichondrial flap, and using high speed drill with cutting bur to remove the deviated part or fracturing and repositioning. Patients were monitored for intraoperative bleeding and followed for at least four weeks regarding the development of upper incisors numbness. The results of this study showed that bleeding was much reduced during surgery and only one patient (2.1%) developed upper incisors anesthesia who have sever maxillary crest deviation that necessitated total excision of the spur. This study gives another option for bony spur correction by providing more controlled excision with less intraoperative hemorrhage and postoperative numbness, this improves patients health and relieves discomfort after surgery.

Abstract

This study was undertaken to design the relationship between different orthodontic cases and amount of bone level loss. The study sample consisted of 72 male students (20–23 years old) classified into 3 groups: Control group with normal overjet and overbite, overjet group with at least 8 mm, and overbite group with at least 6 mm. The results showed a significant reduction of bone height in upper and lower incisors in malocclusion group at significant level 0.01 if it is compared with normal group bone level.

Abstract

Abstract To study the effect of crest shape on the performance of rectangular side weirs, one hundred and four side weir models were tested, eight of them were for sharp crest and ninety six for rounded crest. For each set of rounded crest models, the radius of crest was changed four times and for each radius, the height of the side weir was varied four times. A total of nine hundred thirty four tests were conducted on these models. For all models of different crest shapes, it was found that the average energy difference between two ends of the side weir is very small and less than one percent which can be ignored. The variation of discharge coefficient (CM) with upstream Froude number (Fr1) was found to be linear. The correlation between (CM) and ratio of upstream water depth to weir height ( h1/p ) was found to be linear. The variation of (CM) with the ratio of upstream head above crest to crest radius (h1/r) was also found linear. A simple power empirical expression was obtained for the combined effect of (Fr1) and (h1/p) on (CM) for free flow over sharp crest side weirs with high correlation coefficient. Three simple power empirical expressions were obtained for the variation of (CM) with (Fr1), (h1/p) and (h1/r) for the free flow over inside semicircular, outside semicircular and circular crest shapes with high correlation coefficients. From the hydraulic performance, it was found that small weir heights gave higher performance for all crest shapes. The highest performance of inside semicircular crest was found for crest radius (r) =3.15cm, the highest performance of outside semicircular crest was found for (r) =3.75cm, while, the highest performance of circular crest was found for (r) =2.5cm and (r/p) =0.25.

لدراسة تاثير شكل القمة على اداء الهدارات الجانبية المستطيلة لقد تم اختبار مائة و اربع نماذج لهدارات جانبية ثمانية منها ذوات قمة حادة و الستة و التسعون الاخرى ذوات قمة مستديرة لكل مجموعة من هذه النماذج تم تغير نصف قطر القمة اربع مرات و لكل نصف قطر تم تغير الارتفاع اربع مرات. اجريت تسعمائة و اربع و ثلاثون تجربة على هذه النماذج. لجميع النماذج المختلفة الاشكال ظهر بان معدل فقدان الطاقة على نهايتي الهدار الجانبي صغير جدا واقل من واحد في المائة ويمكن اهماله .تغير معامل التصريف (CM) مع رقم فرود المقدم (Fr1) اتضح بانه تغير خطي و لقد اتضح بان العلاقة بين (CM) و نسبة عمق الماء المقدم فوق الهدار الى ارتفاع الهدار(h1/p) هي علاقة خطية و ان العلاقة بين (CM) و نسبة عمق الماء المقدم فوق الهدار الى نصف قطر القمة(h1/r) هي علاقة خطية كذلك. تم الحصول على علاقة وضعية اسية للتاثير المشترك ﻟ (Fr1) و(h1/p) على (CM) للجريان الحر فوق الهدارات الجانبية ذوات الحافة الحادة و بعامل ارتباط عال. كما وتم الحصول على ثلاث علاقات وضعية اسية لتغير (CM)مع (Fr1) و(h1/p) و(h1/r) للجريان الحر فوق الهدارات الجانبية ذوات القمة نصف الدائرية الداخلية و نصف الدائرية الخارجية و الدائرية و بمعاملات ارتباط عالية. من الاداء الهيدروليكي ظهر بان الهدارات ذوات الارتفاعات الواطئة تعطي اداءا افضل لكل اشكال القمة. وافضل اداء لهدارات القمة نصف الدائرية الداخلية كان عند نصف قطر القمة (r) = 3.15 سم واعلى اداء لهدارات القمة نصف الدائرية الخارجية كان عند (r) =3.75 سم بينما اعلى اداء لهدارات القمة الدائرية ظهر عند (r) =2.5 سم و(r/p) =0.25.

Abstract

Today the world is witnessing very significant developments in the world of digital communications. For that, the subscribers are demanding for quality and speed for the available services. To achieve these objectives, must be accurate and low impurity communication devices available. The orthogonal frequency division multiplexing (OFDM) scheme, is the downlink modulation scheme utilized in the long term evolution (LTE) system. Because of the coherent addition of the subcarriers, the output power variation will show high peaks compared with the average, known as Crest factor, thus, the amplification gain is degraded and intermodulation distortion will appears. Different sorts of schemes have been introduced to mitigate these problems, but either on the expense of complexity or the bit error rate (BER) performance degradation. In this paper, a novel simple to implement algorithm suggested by the authors to mitigate the high Crest factor. The suggested approach is based on complex numbers manipulation, by exchanging the real and imaginary parts, to destroy the coherence order of the OFDM subcarriers, leading to lower crest factor. Results show that the computational complexity and the reduction gain are improved significantly.

Abstract

A semicircle crest profile extending across full width of laboratory channel (suppressed) provides measurements of discharge – head under free flow condition. According to these measurements an empirical head – discharge relationship has been established. Dimensional analysis is carried out to establish independent parameter (R/H) which represents relative crest curvature and dependent parameter (q/〖g^(1⁄2) H〗^(3⁄2) ) . Regression analysis and solver function in Microsoft excel is used to determine the flow equation, to obtain a relationship between depending discharge and the independent parameters affecting the flow. Non-linear equation for estimating the discharge coefficient of the weir model was developed. The accuracy of HEC-RAS software is examined for describing water surface profiles, computation of rating curve and the occurrence of the critical depth .The HEC-RAS program results are compared with the laboratory measurements. Yield affair agreement between them. The weir coefficient of semicircle crest profile is greater than that for the basic broad crested weir and the flow upstream of the weir is subcritical .Transitional flow occurs on the weir crest and the flow along downstream weir face is supercritical within the range of modular discharges.

تم في هذا البحث استخدام سد غاطس ذو حافة نصف دائرية موضوع على عرض القناة المختبرية (غير مقلص) في قياس معدل الجريان المار عبر السد والشحنة في مقدم السد الغاطس تحت ظروف جريان حر. من خلال هذه النتائج المختبرية تم ايجاد معادلة تجريبية ما بين التصريف و الشحنة المقاسة. بأستخدام طريقة التحليل البعدي تم ايجاد علاقة مابين المتغير المستقل (R/H) الذي الذي يمثل (التقوس النسبي للحافة) و المتغير المعتمد اللابعدي q/〖g^(1⁄2) H〗^(3⁄2) . وبتطبيق طريقة الانحدار المتعدد في برنامج المايكروسوفت اكسل تم ايجاد معادلة لحساب التصريف الذي يمثل المتغير المعتمد والمتغيرات المستقلة المؤثرة في الجريان ومعادلة لحساب معامل التصريف بدلالة المتغير المستقل ((R/H.تم تشغيل برنامج (HEC-RAS) لحالة الجريان الثابت و حساب المقاطع الطولية ومنحني المعايرة والعمق الحرج للجريان والتحقق من نتائج البرنامج عند المقارنة مع القراءات المختبرية والتي اظهرت عن توافق جيد .ان معامل السد ذو الحافة النصف دائرية هو اكبر من معامل السد العريض الحافة وهنالك انتقال للجريان من جريان تحت الحرج في مقدم السد الى جريان فوق الحرج في مؤخرة السد مارا بالحالة الحرجة في قمة السد ضمن حدود الحريان الحر.

Abstract

Background and objectives: measuring the distance between the lowest point of the maxillary sinus floor and maxillary alveolar bone crest seen in the digital panoramic images and the effect of alveolar bone resorption to evaluate the clinical benefit of using software measurement program to measure the height of the alveolar bone in the region of maxillary sinus.Method: Digital panoramic images were analyzed using the software computer programs that existed in the system to measure the distance from the lowest point of the maxillary sinus floor and maxillary alveolar bone crest. Images were divided into four groups (1st group dentulous cases, 2nd group single dental extraction, 3rd group multiple dental extraction, 4th group edentulous cases) and each group consist of 50 panoramic images.Results: The mean distance of sinus floor to bone crest in fourth group cases was approximately half of the distance in first group due to extraction of teeth and there was highly significant difference (P<0.001) between first group, third group and fourth group images, and there was significant difference (P<0.005)between first group and second group images.Conclusion: It was concluded that the objective assessment of alveolar bone height and its 2-dimensional relation to maxillary teeth was greatly helpful by applying the software program.

Abstract

The present study was designed to choice the best toothpaste , were using every day to reduce proliferation microorganisms in mouth as well as bad smell of mouth because of presenting microorganism in mouth and were found food between teeth . The antimicrobial activity of seven different toothpastes Emoform, Colgate, Signal , Crest , Sanino, Close up & Sensodyne were evaluated on some pathogenic oral microorganisms : Streptococcus mutans, Micrococcus spp., Proteus vulgaris Staphylococcus aureus & Candida albicans. After wash the volunteers teeth with toothpaste, Emoform toothpaste was more effect than another type by killing of all type of bacteria. The antimicrobial activity was performed by using agar well diffusion method demonstrate that the Emoform toothpaste its more effect than another types. the diameter of inhibition zone was (30, 25, 23, 31, 27 and 35 mm) against C. albicans, S.aureu, Str. Mutans ,Micrococcus spp, P. vulgaris , S. epidermidis and B. subtilis respectively. In conclusion the antimicrobial activity of Emoform, Colgate , Crest were better than Sanino, Sansodyne and Close up.

وقد تم تصميم هذه الدراسة لاختيار أفضل معجون الأسنان، وتستخدم كل يوم للحد من الكائنات الحية الدقيقة انتشار في الفم وكذلك الرائحة الكريهة من الفم بسبب تقديم الكائنات الحية الدقيقة في الفم وعثر على المواد الغذائية بين الأسنان. نشاط مضادات الميكروبات من سبعة معاجين الأسنان المختلفة Emoform، كولجيت، الإشارة، كريست، Sanino، قرب وجرى تقييم سنسوداين على بعض الكائنات الحية الدقيقة المسببة للأمراض الفم: العقدية الطافرة، ميكروكوكس النيابة، المتقلبة الاعتيادية المكورات العنقودية الذهبية والمبيضات البيض. بعد غسل الأسنان المتطوعين مع معجون الأسنان، ومعجون الأسنان Emoform أكثر تأثير من نوع آخر عن طريق قتل من كل نوع من البكتيريا. تم تنفيذ النشاط البكتيري باستخدام أجار طريقة جيدة نشر إثبات أن Emoform معجون الأسنان المزيد من تأثيره من أنواع أخرى. كان قطر منطقة التثبيط (30، 25، 23، 31، 27 و 35 ملم) ضد C. المبيضة البيضاء، S.aureu، شارع. الطافرة، ميكروكوكس النيابة، P. الشائع، البشروية س و ب الرقيقة على التوالي. وفي الختام كان نشاط مضادات الميكروبات من Emoform، كولجيت، كريست أفضل من Sanino، Sansodyne وقرب.
