Detection ESBL enzymes in gram-negative bacteria

Abstract

The prevalence of Extended-spectrum ß-lactamase (ESBLs) enzymes are considered a large problem all over the world. The misuse of broad-spectrum-antibiotics especially in immunocoprmized and hospitalized patients has lead to emergence of antibiotic-resistant strains of Enterobacteriaceae and several Gram-negative bacteria (GNB), for that reason, there's an urgent need to investigate these enzymes by a continuously examination and blood culture of these patients. Blood samples n=147 were collected from Hemodialysis patients that treatment in Basrah General Teaching Hospital and Al-Sader Hospital in Basrah governorate south of Iraq, conventional microbiological methods (Biochemical tests) and also molecular methods by implified 16s rDNA and genetic sequence analysis, were used to dignosis the common types of Gram-negative bacteria and two methods were used, double disc synergy test (DDST) and double disc approximation method (DAM) to detect the present of ESBLs-enzymes in these samples. Twenty seven of Gram-negative bacteria were obtained out of 147 blood samples, and out of 27 GNB, 16(59.3 %) was positive produced ESBLs-enzymes. Cefotaxime (CTX), was found to be the more sensitive to detect the ESBLs-enzymes.

Abstract

From October 2013 to the end of March 2014 , 26 isolates of Pseudomonas aeruginosa were isolated from patients visited Al-Sadr Medical City Hospital suffering from various infections. These isolates were tested using disk diffusion test for susceptibility of imipenem, where five isolates were resistant to this agent, 3 were moderate, while 18 isolates were susceptible. The resistant isolates were submitted to Minimum Inhibitory Concentration (MIC) assay using E-test strips (Dilution and Diffusion method). The MIC values varied from 1-12 µg/ml. Phenotypic detection for Metallo – β – Lacamases enzymes in resistant isolates was done using two tests, the Double Disk Synergy Test (DDST), and Combined Disk Test (CDT). The results showed that three isolates were positive to MBL production by (DDST)at (10) mm , and two at (15)mm , only one isolate was positive by the CDTs giving diameter difference (16) mm.

للفترة من شهر تشرين الاول الى نهاية شهر اذار عزلت 26 عزلة من بكتريا Pseudomonas aeruginosa من مرضى زاروا مستشفى الصدر يعانون من التهابات مختلفة. اجري لهذه العزلات فحص الحساسية لمضاد الاميبينيم. كانت 5 عزلات مقاومة و3 معتدلة والباقية حساسة. اجري بعدها فحص E- test لمعرفة التركيز المثبط الادنى Minimum Inhibitory Concentration (MIC), تباينت نتائج الـMIC بين 1-12 مكغم/مل. تم التحري عن انتاج انزيمات البيتالاكتام المعدنية Metallo – β – Lacamases مظهريا باختباري تآزر القرص المزدوج Double Disk Synergy Test و القرص المتحد Combined Disk. اظهرت النتائج ان 3 عزلات كانت موجبة لانتاج انزيمات البيتالاكتام المعدنية عند مسافة 10 ملم و عزلتين عند مسافة 15ملم وعزلة واحدة فقط كانت موجبة لفحص القرص المتحد بقطر تثبيط قدره 16 ملم.

Abstract

Background and objective: Bacterial resistant to broad spectrum β-lactams, which is mediated by the extended spectrum beta lactamase enzyme, has emerged recently as increasing problem. Extended spectrum beta lactamase producing strains can also displaying multi-drug resistance. Thus, increased number of infections due to these strains is a public health issue associated with high morbidity, mortality, high health-care costs and prolonged hospitalization. Therefore, this study aimed to evaluate multi-drug resistance among extended spectrum beta lactamase producing E. coli causing urinary tract infections. Methods: A total of 400 mid-stream urine specimens were collected from patients with urinary tract infection. Disk diffusion agar method on Muller-Hinton agar plates was carried out. Double Disc Synergy Test was used for detection of extended spectrum beta lactamase producer. All the isolates that were screened out for extended spectrum beta lactamase production were also subjected to confirmation by using the Phenotypic Confirmatory Combination Disc Diffusion Test.Results: The urinary tract infection cases were mainly due to Gram negative bacteria (87%). E. coli was isolated from 195 (48%) specimens. Sixty isolates of E. coli (31%) were found to be extended spectrum beta lactamase producers. The resistance to antibiotics tested was significantly higher (P <0.001) among extended spectrum beta lactamase producing isolates. Conclusion: The prevalence of multi-drug resistance to the antibiotics among extended spectrum beta lactamase producing E. coli isolates was established. Imipenems are recommended for the treatment of serious infections caused by these organisms.
