The Role of Resistin in Patients with Primary Fibromyalgia

Abstract

Background: Fibromyalgia syndrome (FMS) is a common rheumatologic syndrome with multiple manifestations and associated with many diseases, it characterized by chronic wide spread muscular pain and tenderness. Resistin is an adipokine discovered in 2001 and considered as a link between obesity and type 2 diabetes by antagonizing and resisting the action of insulin as it name encompass ;resistin (resist insulin) . It is secreted mainly from adipose tissue in mice and from white blood cells besides the adipose tissue in humans. Research reviled that resistin has a role in some inflammatory diseases like Rheumatoid Arthritis (RA).
Objective: The objective of this study is to measure Resistin concentration and to evaluate its role in the pathogenesis of primary FMS.
Patients and methods: Fifty patients with primary FMS were included in this study (37 females and 13 males) the age range of (17-65) years (Mean ± SD) (40.13 ± 12.0) years, and thirty healthy individuals volunteers (21 females and 9 males), whose age and sex matching with FMS patients, age ranging (18 - 63) years, (Mean ± SD) (36.1 ±10.0) years. Resistin concentration was measured in sera of patients and controls by ELISA kit. Anthropometric measurements like body mass index (BMI) and waist circumference (WCr) were taken, besides other features like sleep disturbance, emotional distress, and fatigue were reported.
Results: Resistin concentration was higher in FMS patients than controls (2.15±0.9) ng /ml vs. (1.57±0.6) ng /ml. This elevation was highly significant statistically (p=0.003).Other measurements in patients group like BMI, WCr were (28.39±5.0) Kg/m2 and (100.34±13.21) cm respectively .These values were highly significant when compared to their control group (p=0.002) and (p=0.008) respectively . Serum resistin level was positively correlated with both waist circumference (r=0.411,p=0.003) and age (r=0.405 ,p=0.004) . Waist circumference was positively correlated with age (r=0.604, p=0.000).Clinical features like sleep disturbance, emotional distress ,and fatigue showed highly significant difference between the two groups . No significant differences were reported with respect to age and sex.
Conclusion: The result of the current study suggest that Resistin might have a role in the pathogenesis of FMS.

Abstract

Background: Fibromyalgia has been associated with physical and emotional trauma including invasive medical procedures. Both Fibromyalgia and ischemic heart disease have been linked with depression. The aim of the study is to assess the prevalence of fibromyalgia syndrome in patients with ischemic heart disease.Patients and Methods: A cross-sectional comparative study was done in the Iraqi Center for Heart Disease. One hundred patients, angiographically proved to have ischemic heart disease, were subjected to rheumatologic examination by another physician, and compared to hundred healthy individuals as a control group. Diagnosis of fibromyalgia was based on 1990 American College of Rheumatology Classification criteria for the diagnosis of fibromyalgia.Results: Eighteen (18%) patients with ischemic heart disease fulfilled the American College of Rheumatology criteria for fibromyalgia syndrome in comparison to only 6(6%) of the controls, and this difference is statistically significant (P-value = 0.009).There were statistical significant effects of gender, age, and number of coronary vessels on the prevalence of fibromyalgia syndrome. Conclusion: fibromyalgia occurs with high frequency (18%) in patients with ischemic heart disease.

Abstract

Fibromyalgia (FM) is a common, debilitating, and chronic pain syndrome. The women are more likely to have more tender points on examination than are their male counterparts. Iraqi study showed that FM occur in 1.5% among adolescents of Iraqi population. In compare to normal healthy women, present study was revealed that Iraqi women with FM have significant elevation of calcium (p = 0.003) with significant reduction of magnesium (p = 0.001), whereas the inorganic phosphorous was not differs (p = 0.31). In conclusion, magnesium and calcium would play a crucial role in etiopathogenesis of fibromyalgia.

الفیبرومیلاجیا او متلازمة ألم اللیف العضلي ھي متلازمة الم مشترك, منھك و مزمن. النساء أكثر عرضة للمزید من نقاطالوھن اثناء الفحص السریري من نظرائھن من الرجال .وأظھرت دراسة عراقیة ان الفیبرومیلاجیا تحدث في 1,5 ٪ من البالغین منالعراقیین . بالمقارنة مع نساء غیر مصابات , اظھرت الدراسة الحالیة ان النساء العراقیات المصابات بالفیبرومیلاجیا یظھرن ارتفاعافي المصل. بینما لم یظھرن اختلافا (p = وانخفاضا معنویا بمستوى المغنیسیوم ( 0.001 (p = معنویا بمستوى الكالسیوم ( 0.003یمكن الاستنتاج من نتائج الدراسة انھ من الممكن أن یكون للكالسیوم والمغنیسیوم .(p = معنویا بمستوى الفسفور غیر العضوي ( 0.31دورا ھاما في امراضیة الفیبرومیلاجیا .

Serum Lipoprotein (A) in Patients with Fibromyalgia Syndrome

Abstract

Background: Fibromyalgia syndrome (FMS) is the most common rheumatic cause of diffuse pain and multiple regional musculoskeletal pain and disability. Objective: is to assess the contribution of serum lipoprotein (A) in the pathogenesis of FMS patients.Methods: One hundred twenty two FMS patients were compared with 60 healthy control individuals who were age and sex matched. All FMS features and criteria are applied for patients and controls; patients with secondary FMS were excluded. Serum Lipoprotein (A): [Lp(A)], body mass index (BMI), & s.lipid profile were determined for both groups.Results: There was a statistical significant difference between patients &controls in serum lipoprotein (A) (P=0.013). Also there was a statistical significant correlations between serum Lp(A) & FMS patients' age( r= 0.310, P=0.034 ); but not with: duration(r= -0.222, P=0.133) , BMI(r= 0.128,P=0.390) & s.lipid profile ( p> 0.05) of FMS patients.Conclusion: s.lipoprotein (A) may play an important role in pathogenesis of FMS patients.

Abstract

ABSTRACT:BACKGROUND:Joint hypermobility occur when large or small joints in the body are able to extend beyond theirnormal physiological limits. Fibromyalgia is characterized by chronic diffuse pain. Both conditionsare shared with various soft tissue problems and this draw the attention of a lot of researchers andclinicians to look for possible relations between them.OBJECTIVE:To detect the correlation between firbomyalgia in Iraqi people with joint mobility.METHODS:One hundred Iraqi individuals with joint hypermobility syndrome according to the Beighton Scorewere studied and compared with another hundred normal mobile individuals matched for age and sexserving as control group. Full history was taken and complete clinical examination was done for allindividuals in both groups. The American College of Rheumatology 1990 Criteria for fibromyalgiawere applied for all individuals in both groups. They were quationed about presence of chronicwidespread pain, sleep disturbance, mood change and fatigue. Detection of at least 11 of 18 tenderpoints by digital palpation at specific soft tissue sites was needed for diagnosis of FMS.RESULTS:There was statistically significant increase frequency of fibromyalgia among individuals withhypermobile joints compared to normal mobile people. The frequency was 22% in the individualswith hypermobile joints versus 11% in the individuals with normal mobile joints, which was reportedmainly in females between the age 30-59 years.CONCLUSION:The results showed significant relationship between fibromyalgia and joint mobility.

Further, circulatory levels of pro-inflammatory cytokines like IL-8 and others may be altered in FMS patients, possibly associated with their symptoms.The objective of the study is to measure IL-8 concentration and to evaluate its role in the pathogenesis of primary FMS.OBJECTIVE:Fifty patients with primary FMS were included in the study (37 females and 13 males) the age range of (17-65) years (Mean ± SD) (40.13 ± 12.0) years, and thirty healthy individuals volunteers (21 females and 9 males), whose age and sex matching with FMS patients, age ranging (18 - 63) years, (Mean ± SD) (36.1 ±10.0) years.METHODS:IL-8 concentration was measured in sera of patients and controls by ELISA kit. No significant differences were reported with respect to age and sex.CONCLUSION:The result of the current study suggest that interleukin-8, (IL-8) might have a role in the pathogenesis of FMS.

Abstract

ABSTRACT:BACKGROUND:Fibromyalgia syndrome is a common rheumatological syndrome with multiple systemic manifestations which could occur alone or associated with many diseases.OBJECTIVE:To detect the possible relation between fibromyalgia and haemophilia-A patients.PATIENTS AND METHODS:One hundred patients with haemophilia-a were studied and compared with another hundred, healthy individuals matched for age and sex, serving as control group. Full history was taken and complete clinical examination was done for all individuals in both groups.The american college of rheumatology 1990 criteria for fibromyalgia were applied for both groups. They were questioned about presence of chronic widespread pain, sleep disturbance, headache fatigue and parasthesia . Detection of at least 11 of 18 tender points by digital palpation of specific soft tissue sites were needed for diagnosis of fibromyalgia.RESULTS:There was a significant increase in the frequency of fibromyalgia among individuals with haemophilia-a (25%) compared to healthy control group (3%) (p=0.000004, odd ratio=0.093, 95% confidence interval 0.027-0.319).CONCLUSION:Fibromyalgia occurs with high frequency in hemophilia A patients

Abstract

ABSTRACT:BACKGROUND: Ankylosing Spondylitis is a chronic systemic inflammatory disorder affecting the spine and other areas of the body(joints and entheses). Fibromyalgia is a syndrome characterized by chronic( more than three months) wide spread musculoskeletal pain. Detection of fibromyalgia in patients with ankylosing spondylitis is an important aspect in the treatment of ankylosing spondylitis. OBJECTIVE: To determine the association of fibromyalgia with adult ankylosing spondylitis, and to evaluate the effect of coexisting fibromyalgia on the scores of disease activity and functional status.PATIENTS AND METHODS:A case-control study with 100 ankylosing spondylitis patients and 100 controls enrolled in the study. Ankylosing spondylitis was diagnosed with modified New York criteria. Fibromyalgia assessed by using modified 2010/ 2011 American Collage of Rheumatology criteria for the diagnosis of fibromyalgia. RESULTS: Fibromyalgia was found in 25% of patients with ankylosing spondylitis with female: male ratio(5.2:1), while seen in 6% of controls with female to male ratio(5.9:1). Fibromyalgia was significantly associated with increase in the disease activity index. The function index in fibromyalgia patients was significantly higher than in non-fibromyalgia patients.CONCLUSION: Fibromyalgia is more common in ankylosing spondylitis patients than in controls. The coexistence of fibromyalgia with ankylosing spondylitis negatively impacts the scores of ankylosing spondylitis disease activity and functional status.KEYWORDS: Fibromyalgia, Ankylosing spondylitis.

Abstract

ABSTRACT: BACKGROUND: Fibromyalgia syndrome (FMS) is a common rheumatologic syndrome with multiple systemic manifestations & associated with many diseases. OBJECTIVE: To assess fibromyalgia syndrome in inflammatory bowel disease. PATIENTS AND METHODS: One hundred four Iraqi patients with inflammatory bowel disease were studied and compared with another (112) healthy individuals matched for age and sex as a control group. Full history was taken and complete clinical examination was done for all individuals in both groups. Fibromyalgia syndrome was diagnosed in all patients on base of the American College of Rheumatology (ACR) 1990 Criteria for the classification of FMS. Inflammatory bowel disease was detected by colonoscopy & tissue biopsy. RESULTS: There was a significant increase in the frequency of FMS among individuals with IBD (24 %) compared to healthy control group (5.4%) ((P-value =0.0001, Odd ratio=0.18, 95% CI=0.07-0.46). CONCLUSION: FMS occurs with increased frequency in IBD.

Abstract

ABSTRACT:BACK GROUND:Fibromyalgia is a disorder characterized by widespread musculoskeletal pain, fatigue, sleep and mood disturbances. Its etiology and treatment remain challenging. Oxidative stress may play a role in its pathogenesis.OBJECTIVE:The efficacy of pregabalin to that of amitriptyline on FM and the effect of treatment on oxidative status were examined.METHODS:123 patients fulfilling the Wolfe 2010 criteria for fibromyalgia diagnosis and assessment were randomized to receive amitriptyline (25 mg) or pregabalin (75 mg) once daily for 12 weeks. The revised fibromyalgia impact questionnaire criteria were also used. Malondialdehide and glutathaion levels were checked at the beginning and at the end of the study.RESULTS: Improvement in the pain was better with pregabalin than with amitriptyline (P=0.0001) at 4 weeks of treatment, but achieved comparable improvement at 8 and 12 weeks. The reduction in the somatic symptoms scale was significant for both drugs in favour of pregabalin. Sleep was improved significantly but pregabalin over amitriptyline (P=0.0001). Oxidative status was significantly improved.CONCLUSION:Both drugs improved the symptoms. Pregabalin was better than amitriptyline concerning SSS and sleep, but with a comparable effect on pain. Oxidative status was improved. Amitriptyline was better than pregabalin concerning patient compliance and tolerability.
