Immunohistochemical expression of P53, as a marker of apoptosis in Hodgkin’s and Non Hodgkin’s lymphoma of the head and neck region

Abstract

Background: Malignant lymphomas represent about 5% of all malignancy of the head and neck region which caninvolve lymph nodes as well as soft tissue and bone of the maxillofacial region. Apoptosis is considered a vitalcomponent of various processes including normal cell turnover, proper development and functioning of the immunesystem. Inappropriate apoptosis is a factor in many human conditions including neurodegenerative diseases,ischemic damage, autoimmune disorders and many types of cancer. Expression of p53 Proteins in Hodgkin ׳s and NonHodgkin ׳s lymphomas suggested that it can help in monitoring of patients and the markers may aid in controlling theprogression of lymphoma and detect the degree of aggressiveness of the disease to give suitable treatment andmanagement of patients.Material and methods: Sixty seven formalin-fixed, paraffin-embedded tissue, histopathologically diagnosedlymphoma blocks (head and neck lesions) as (24) Hodgkin’s Lymphoma and (43) Non Hodgkin’s Lymphoma.Immunohistochemical (IHC) technique was used for the evaluation of P53 monoclonal antibodies expression andcorrelated with the clinicopathological parameters.Results: The mean of expression of P53 in relation to tumor grades was different as it had a value of (51.2±13.5) in lowgrade tumors, (55.7±20) in the intermediate grade tumor and (45±17.6) in high grade tumors; consequently, thisdifference did not reached the level of statistical significance P(ANOVA) >0.05.Conclusions: This study had shown that there was no significant correlation between the mean of expression of P53 inHL and NHL

خلفیھ:اللمفوما الخبیثھ تمثل حوالي 5% من كل الامراض لمنطقة الرأس والعنق والتي یمكن ان تنطوي على العقد اللمفاویھ وكذلك الانسجھ الرخوه ومنطقة الوجھ والفكین. موت الخلایاالمبرمج یعتبر عنصر حیوي لكثیر من العملیات المتنوعھ من ضمنھا عملیة التحول الطبیعي للخلایا والتطور المناسب وعمل الجھاز المناعي. موت الخلایا المبرمج الغیر مناسب (امابصوره قلیلھ أو كثیره) ھو عامل في العدید من الحالات البشریھ من ضمنھا أمراض التحلل العصبي، التلف نتیجھ فقر الدم الموضعي ،حالات أضطراب المناعھ الذاتیھ وعدة أنواع منP في ھودجكن ولاھودجكن لمفوما مستعملین طریقة النسیجي المناعي الكیمیائي ، اقترحت بأن الدراسات المناعیھ النسیجیھ الكیمیائیھ لعامل 53 P السرطان. التعبیر عن بروتین 53وظھوره في أورام الانسجھ یمكن أن یساعد في رصد المرضى والدراسات وكذلك المعلمات ممكن أن تساعد في ضبط تقدم اللمفوما ونتحقق من درجة عدوانیة المرض لاعطاء العلاجالملائم وأدارة المرض.المواد والطرق: سبعھ وستین نسیج مثبت بالفورمالین ومطموره بشمع البرافین، مشخصھ نسیجیا من منطقة الراس والعنق.اربعھ وعشرون حالة ھودجكن لمفوما وثلاثھ وأربعونكمضاد احادي النسل وربط ذلك مع المعاییر العیادیھ المرضیھ. P لاھوجكن لمفوما. تقنیة المناعھ الكیمیائیھ النسیجھ قد استعملت لتقییم تعبیر 5317.6 في الورم ± 20 في الورم متوسط الدرجھ و 45 ± 13.5 في الورم واطيء الدرجھ و 55.7 ± وعلاقتھ بدرجھ الورم كانت مختلفھ كما كانت لھا قیمھ 51.2 P النتائج: معدل تعبیر 53. P> عالي الدرجھ: وكنتیجة الاختلاف لم یصل الى المستوى الاحصائي المھم 0.05في ھودجكن ولاھوجكن. P الاستنتاج: ھذه الدراسھ قد أظھرت بأنھ لم تكن ھناك علاقھ مھمھ بین معدل تعبیر

Abstract

Abstract:Background: Hodgkin disease (HD) is a histologically defined B- cell neoplasm and it includes two distincttypes of disease, classical and nodular lymphocyte-predominant Hodgkin’s disease. Disruption of thephysiological balance between cell proliferation and apoptosis is a universal feature of all cancers.Apoptosis is caused by activation of the caspases through extrinsic and intrinsic pathways. Extrinsicpathway centers on TNF family, where the ligand will bind to the cell surface receptor to induceapoptosis. Fas receptor is a member of the TNF receptor superfamily. Fas family is constituted of thereceptor, ligand and soluble form. Soluble Fas will interfere with apoptosis by competing with Fasreceptor for binding to legend. The aim of this study is to measure the concentration of serum soluble Fasin Hodgkin’s disease patients and to determine the relation between it and certain clinical parameters andserum markers. Also to compare the concentration of serum sFas in HD and Non-Hodgkin’s lymphoma(NHL) patients.Patients & Methods: This study included 15 patients with Hodgkin's disease, 9 males and 6 females. Thepatients were interviewed for history and clinical examination and blood was collected for measurementof serum sFas concentration using ELISA technique. Hemoglobin (Hb) concentration, leucocytes andplatelets counts, peripheral blood smear, C-reactive protein (CRP) and lactate dehydrogenase (LDH) levelwere all performed using standard techniques.Results: This study revealed that the serum soluble Fas concentration was almost the same (p-value = 0.991) inboth HD patients (1996.9 ± 131) and normal controls (1993 ± 125.9). However, by comparing the resultsof this study with the high concentration results obtained for serum sFas in newly diagnosed NHLpatients (6475.9 ± 617) of another study, it revealed a highly significant difference (p-value < 0.0001)between the two groups.Conclusions: The serum Fas concentration differs significantly between HD and NHL patients but not withcontrol patients. Further studies including larger number of patients are recommended.

Abstract

Background: Interleukin-10 (IL-10) is a pleiotropic cytokine produced by type 2 helper cells (TH2), monocytes ,macrophages, and neoplastic B lymphocytes IL-10 production has a strong immunosuppressive effect, and it actsas an effective stimulator for B-cells .IL-10 level increase in Non-Hodgkin lymphoma (NHL) patients ;moreover ,this was associated with poor prognosis .Objectives: to assess the level of interleukin-10 in patients with non-Hodgkin lymphoma and to correlate its level withprognosis.Materials and Methods: The present study was carried out to evaluate IL-10 level in the sera of 46 NHL Iraqi patients,in addition to a control group involving 46 matched apparently healthy subjects using serological method theenzyme-linked immunosorbent assay (ELISA).This study was carried out at the National center of hematologyfrom January to June 2013.Results: there were 31 (67%) patients with age group ≥50 years and 15 (33%) patients with age group <50 years. Themean age of NHL patients was 52.74 years ranging from 17 to 80 years? Moreover, the study noticed a malepredominance and male to female ratio was 1.7:1.When the patients of NHL compared with control, the levels ofinterleukin 10 were markedly increased in patients with NHL (18.67 ± 4.12pg/ml vs. 3.4 ± 0.73pg/ml) (p<0.05).Higher levels of IL-10 were noticed in the advanced stage patients of compared to early stages (33.1 ± 4.5 pg. /mlvs. 13.4 ± 2.7 pg. /ml) (P<0.05).Conclusion: In the highest age of NHL patients was noticed and it might indicate the increasing rates of NHL patientsamong older population. in addition increasing level of IL10 is associated with increasing age range and alsocorrelated positively with aggressive NHL cases that might indicates the predictive pivotal role of this cytokine.

Abstract

Background: Malignant lymphomas in the head and neck region are relatively uncommon but may occur as a part of widely disseminated disease or as a primary lesion. This study was carried out in an attempt to evaluate the incidence of Non-Hodgkin's Lymphoma of head and neck in Baghdad city over ten years (1990-1999).Materials and methods: Cases of Non Hodgkin’s Lymphoma of head and neck in Baghdad city were collected and evaluated regarding sex, age, site and histopathological subtype of tumor.Results: Out of the total of (426) cases of Non-Hodgkin's Lymphoma of the body, (137) were showed to affect the head and neck region including 78 males and 59 females.Conclusions: Our findings showed that the principle site affected with Non-Hodgkin's Lymphoma was the cervical lymph nodes followed by the tonsils with the predominant histopathological subtypes was the lymphocytic Lymphoma followed by Reticular cell sarcoma.

Abstract

ABSTRACT:BACKGROUND:This work presents an epidemiological approach used to deal and study Hodgkin’s lymphoma (HL) in the Iraqi patients.METHODS:This study was carried out for 80 newly diagnosed HL patients (32 females and 48 males), compared with 50 Non Hodgkin lymphoma (NHL) patients control and 50 healthy individuals considered as healthy control in period between 1st of Jun 2003, and 30th of November, 2004. Newly diagnosed HL histopathological samples were taken and diagnosed as HL according to the National Cancer Institute Working Formula. All patients were subjected to personal interview using especially designed questionnaire format.RESULTS:In this study the age of HL patients ranged between 10-73 years with a mean age of (29.31 ± 1.73) years. There was slight males predominance among HL patients 48 (60%) than females 32 (40%). The age incidence below (45) years old forming 67 (84%), versus13 (16%) incidence above 45 years.The cervical lymph node enlarged was commonly found (70%) and the main clinical presentation was of A symptom (27%) in HL patients. The highest subtype number was recorded in mixed cellularity (MC) (61%) and stage II (38%). There was no significant association between cigarette smocking and risk of HD (p>0.05).CONCLUSION:Such epidemiological study can be performed at any hospital especially in hospitals were lymphoma cases is increased.

Abstract

Background: Hodgkin's lymphoma (HL), formerly called Hodgkin's diseases is an uncommon form of lymphoma. Theincidence of Hodgkin's lymphoma shows marked heterogeneity with respect to age, gender, race, geographicarea, social class and histological subtype. This study was carried out in an attempt to evaluate the incidence ofHodgkin's lymphoma of head and neck in Baghdad city.Materials and Methods: The diagnosed cases of Hodgkin's lymphoma of head and neck region in Baghdad citybetween (1990-1999) were collected and analyzed according to age, gender, site and the histopathologicalsubtypes of the tumor.Results: Out of (702) cases of Hodgkin's lymphoma of ten years between (1990-1999),(362 ) of them were occurred inthe head and neck region including (202) males and (160) females. The remaining (340) cases were occurred inother lymph nodes of the body.Conclusion: This study revealed that predominant histopathological subtype of Hodgkin's lymphoma is the mixedcellularity type that showed more predominance in male especially among young age groups and low incidencewith advancing age

Abstract

A prospective study done over a period of 12 months on 30 patients aged 15 to 70 years with initial tissue biopsies of NHL who were seen at the Baghdad Teaching Hospital. Trephine and aspiration BM biopsy were performed with routine evaluation including CBC. All biopsies were classified according to the commonly used classification system, international Working Formulation.Patients are staged according to the Ann-Arbor staging system. Intermediate grade NHL (IGL) comprised the most common type of NHL (43.3%); high grade NHL (HGL) comprised (36.7%); low grade NHL (LGL) comprised the least common type (20.0%).BM was involved in 10 patients out of 30 patients with NHL (33.3%) LGL exhibited a higher incidence of involvement (50.0%) than did patients with IGL (30.8%) and HGL (27.2%),.Hematological abnormalities were observed frequently with and without BM

دراسة مستقبلية اجريت للفترة من بداية شهر اذار سنة 2010 م ولغاية نهاية شهر اذار 2011 م. تم اخذ عينة مؤلفة من ثلاثون مريضا تتراوح اعمارهم ما بين 15-70 سنة مصابين بورم لمفاوي لاهوجكني, ادخلوا الى مستشفى بغداد التعليمي- مدينة الطب- قسم الأمراض الباطنية.أخذت معلومات سريرية دقيقة وتم اجراء فحوصات سريرية ومختبرية دقيقة, كما اجري لهم فحص نسيجي لخزعة نخاع العظم وصنف المرضى الى مراحل مختلفة من المرض وفقا لنظام آن- آربورالعالمي.أظهرت الدراسة النتائج التالية:•وفقا لدرجة شدة المرض :- ( %34,3 ) كـــــانوا ذو درجة متوسطة, (36,7 %) ذو درجة عالية, (20 %) ذو درجة واطئة.•عشرة مرضى من أصل ثلاثين (33,3 %) أصيب نخاع العظم بالخلايا الخبيثة للمرض ومعظمهم كانوا ذو درجة واطئة من الشدة (50 %).

Abstract

ABSTRACTHodgkin's lymphoma ( HL) is uncommon malignant tumor of the lymphatic system,some were shown to contain clonal, Epstein-Barr virus (EBV). Non Hodgkinslymphoma (NHL) are associated with EBV, ZEBRA protein plays a dominant role inthe switch from latent cycle to productive infection of the virus. To access theexpression of ZEBRA protein in Hodgkin's and non Hodgkin's lymphoma patientsand its impact on P53 expression. A total of 28 of patients newly diagnosed to have HLand NHL investigated for EBV ZEBRA protein and p53 expression byimmunohistochemistry technique. p53 was expressed in 80.9% and 100% of malesand females respectively. In this study ( 86.7%) of HL and 88.6%of NHL cases werep53 positive. 87.5% of mixed cellularity were p53 positive. While 83.3% intermediategrade were p53 positive .No significant difference found between p53 expression andgender, type of lymphoma but there is significant difference with histopathologicaltype. In regard to ZEBRA protein ,it is found that all the males and females with HLand NHL cases under study showed ZEBRA positivity.Also the 16 cases of the mixedcellularity type and all the 12 cases of intermediate grade are ZEBRA positive.No significant difference found between ZEBRA expression and gender, but there issignificant difference with type of lymphoma and the histopathological type. Thisstudy shows that there is interaction between ZEBRA and p53 which depend on therelative amounts of those two proteins as confirmed previously and the expression ofZEBRA protein in lymphoma reflects the immunocomprimized state of patients andconfirmed that EBV contribute to the development of malignancy.Key words : Hodgkin's lymphoma, Non Hodgkin's lymphoma, p53 tumor suppressor, ZEBRA protein.

الخلاصةتعتبر الأورام اللمفیة لام ا رض الهوجكن واللاهوجكن لیمفوما من الأورام الخبیثة والتى تت ا رفق مع وجودیلعب دوراً مهماً داخل الخلایا المصابة اثناء خروجه من الطور ZEBRA الفایروس ابشتاین بار، إن البروتینفى خلایا الأورام تحت ZEBRA الكامن الى المتكاثر. هدفت الدراسة الى قیاس نسبة التعبیر الخلوي للبروتینشملت هذه الدراسة ثمإنیة وعشرون مریضا من مرضى الهوجكن ،p الدراسة و تأثیره على تعبیر بروتین السیطرة 53.p و 53 ZEBRA واللاهوجكن لیمفوما بإستعمال طریقة التصبیغ المناعى الخلوي لفحص نسبة التعبیر للبروتین100 فى الذكور والإناث على التعاقب. ووُجد أن %, ھي% 80.9 p كانت نسبة التعبیر الایجابي لبروتین السیطرة 53نسبة التعبیر لنفس البروتین هي % 86.6 في مرضى الهوجكن و% 84.7 في مرضى اللاهوجكن لیمفوما. كإنتIntermediate و % 83.3 في Mixed cellularity تصل الى% 87.5 فى p نسبة التعبیر للبروتین 53و نوع الجنس و نوع اللیمفوما ولكن هناك فرق معنوي p ولم یكن هناك فرق معنوي بین تعبیر بروتین 53 gradeموجبة في الذكور و الإناث وفي نوع اللیمفوما ZEBRA مع نوع النسیج المرضي. كانت نسبة التعبیر للبروتینو نوع الجنس ولكن هناك فرق ZEBRA ونوع النسیج المرضي. ولم یكن هناك فرق معنوي بین تعبیر بروتینمعنوي مع نوع اللیمفوما ونوع النسیج المرضي. بینت نتائج هذه الدراسة وجود علاقة وتداخل بین البروتینفي مرضى اللیمفوما ZEBRA كما إن التعبیر الموجب للبروتین ،P وكذلك بروتین السیطرة المناعیة 53 ZEBRAیؤكد علاقة هذا الفایروس مع هذه الأورام.

Abstract

Background: Lymphomas are a group of diseases caused by malignant lymphocytes that accumulate in lymphnodes and cause the characteristic clinical features of lymphadenopathy. Intercellular adhesion molecule-1 (ICAM-1) (CD54) is a transmembrane glycoprotein belonging to the immunoglobulin superfamily of adhesion molecules.Cortactin was first identified as one of the major substrates for src kinase. because it localized to Cortical actinstructures, The aims of this study was to evaluate and compare the immunohistochemical of ICAM-1 expression ascell adhesion molecule marker and Cortactin expression as invasive marker.Material and Methods: This study was performed on (68) formalin-fixed, paraffin-embedded blocks,histopathologically diagnosed as lymphoma (head and neck lesions).Immunohistochemical staining of ICAM-1andCortactin was performed on each case of the study sample.Results: The expression of ICAM-1 was membranous and cytoplasmic, the study cases showed a 98.5% positivereaction to ICAM-1, score 2 was the most common and found in 69.1% of all cases.The expression of Cortactin wascytoplasmic, 98.5% of cases expressed positive reactions to Cortactin, score 1 was the most common and found in42.7% of all cases.Conclusion: Although the two markers showed a higher expression rate in all lymphomas (both HL and NHL) in thisstudy, they can't be used to differentiate between them, nor can be used to differentiate between the subtypes ofboth HL and NHL. The high ICAM-1positive expression clarified that in addition to its role in cell-cell and cell-stromalinteractions, it participates in proliferation, differentiation and invasion of malignant lymphoma cells.The presentstudy is the first one that used Cortactin as an invasive marker for lymphoma.

Abstract

Background: Malignant lymphoma is a term that describes primary tumors of the lymphoreticular system, almost all ofwhich arise from lymphocytes.MMP-1 is the most ubiquitously expressed interstitial collagenase, a subfamily of MMPsthat cleaves stromal collagens. It is also called collagenase-1.TIMPs which inhibits MMP activity and thereby restrictextracellular matrix breakdown, TIMP-1 is a stromal factor that has a wide spectrum of functions in different tissues.Material and Methods: This study was performed on (68) formalin-fixed, paraffin-embedded blocks,histopathologically diagnosed as lymphoma (head and neck lesions). Immunohistochemical staining of MMP1andTIMP1 was performed on each case of the study sample.Results: The expression of MMP1was cytoplasmic, the study cases showed a 98.5% positive reaction to MMP1, score 3was the most common and found in 60.3% of all cases.The expression of TIMP1was cytoplasmic, 92.6% of casesexpressed positive reactions to TIMP1, score 1 was the most common and found in 57.4% of all cases.Conclusion: This study showed for the first time the effect of MMP-1 in HL, which is considered to be as an invasiveand migratory cell marker. A significant difference was found among the subtypes of NHL in relation to TIMP1,TIMP1inhibits the effect of MMP1 and as MMP1 is elevated the TIMP1 will be elevated too
