Study of Liver and Kidney functions in non-pregnant, pregnant and preeclamptic women
دراسة وظائف الكبد والكلى عند النساء غير الحوامل و الحوامل والحوامل المصابات بتسمم الحمل

Abstract

Three groups of subjects have been divided (25/group): healthy normotensive non-pregnant women (Group A), normal normotensive pregnant women (Group B), and women with preeclampsia (Group C).The levels of serum alanine aminotransferase (ALT), aspartate aminotransferase (AST), total bilirubin , creatinine , blood urea nitrogen, triglyceride , total cholesterol and glucose have been estimated in all subjects. All measured parameters were determined by spectrophotometric analysis. The results showed a significant(P<0.05) increase in serum ALT, AST, blood urea nitrogen, triglyceride and total cholesterol levels in group B as compared to group A. However creatinine, total bilirubin and glucose levels did not show any statistical significant alterations. The women in group C showed to be associated with a significant (P<0.05) rise in all measured parameters except total bilirubin as compared to women in group A. The preeclamptic women (group C) showed significant (P< 0.05) increase in serum alanine aminotransferase , blood urea nitrogen , triglyceride , total cholesterol and glucose levels as compared to women in group B. While serum aspartate aminotransferase, total bilirubin and creatinine levels did not show any statistical significant alteration.

تم تقسيم النساء المشتركات في هذة الدراسة الى ثلاثة مجاميع ، المجموعة الاولى ( (Aتضم النساء الاصحاء من غير الحوامل ذوات ضغط الدم الطبيعي ، المجموعة الثانية (B) تضم النساء الاصحاء من الحوامل ذوات ضغط الدم الطبيعي و المجموعة الثالثة (C ) تضم النساء الحوامل المصابات بتسمم الحمل ، تضم كل مجموعة 25 امرأة. تم قياس مستويات الانزيم ألنين- أمينوترانسفيريز ،الانزيم اسبارتيت- أمينوترانسيفيريز، البيلروبين الكلي ، الكرياتنين ، اليوريا ، الجليسريد الثلاثي ، الكوليستيرول الكلي و الكلكوز في مصل الدم لكل امرأة في المجاميع الثلاثة. تم حساب مستويات كل العوامل المقاسة اعلاه بواسطة تحليل المطياف الضوئي. وكانت النتائج المستحصلة من هذه الدراسة كما يلي:هناك زيادة معنوية) P< 0.05) في متوسط نسبة كل من الانزيم ألنين- أمينوترانسفيريز ،الانزيم اسبارتيت- أمينوترانسيفيريز، اليوريا ، الجليسريد الثلاثي و الكوليستيرول الكلي في مصل دم النساء الحوامل الاصحاء مقارنة بالنساء الاصحاء غير الحوامل ، بينما لم تظهر نتائج الكرياتنين ، البيلروبين الكلي و الكلكوز فروقات معنوية عند مقارنة نسبهم في النساء الحوامل الاصحاء مع النساء الاصحاء غير الحوامل.اما في مجموعة النساء الحوامل المصابات بتسمم الحمل فقد اظهرت زيادة معنوية ( (P< 0.05في متوسط نسبة كل القياسات المعتمدة في الدراسة ماعدا البيلروبين الكلي مقارنة بالنساء الاصحاء من غير الحوامل. اما في مجموعة النساء الحوامل المصابات بتسمم الحمل فقد اظهرت النتائج زيادة معنوية ( (P< 0.05 في متوسط نسبة الانزيم ألنين- أمينوترانسفيريز، اليوريا ، الجليسيريد الثلاثي ، الكوليستيرول الكلي و الكلكوز في مصل الدم عند مقارنته بالنساء الاصحاء من الحوامل. بينما لم يظهر متوسط نسبة الانزيم اسبارتيت- أمينوترانسيفيريز ، البليروبين الكلي والكرياتنين فروقات معنوية عندما تمت مقارنته مع النساء الاصحاء من الحوامل.

Therapeutic Response of Serum Lipids to Atorvastatin in Type II Diabetic Patients

Abstract

Lipid disorders and cardiovascular disease (CVD) risk are known to be increased in patients with diabetes mellitus. The effects of statins on serum lipid levels are well known; however, previous studies did not compare the effects of statins on serum lipid levels in diabetic patients with non-diabetic patients. To investigate the effects of Atorvastatin on serum lipid profiles in hyperlipidemic patients with type 2 diabetes mellitus in comparison with hyperlipidemic patients without diabetes.This study was conducted on 33 type 2 diabetic patients & 34 non-diabetic patients; their age range was 40-80 years, all of them were hyperlipidemic, who had been administered 10, 20, & 40 mg daily of Atorvastatin and completed a 6-month follow-up. Serum lipids were measured before and after treatment at 1, 3, and 6 months.It was found that the reduction in S. Total Cholesterol and S. LDL-C were less in diabetic patients than that in non-diabetic patients when they are using the same doses of Atorvastatin, while the changes in S. HDL-C and S. Triglyceride were nearly similar in both. Furthermore, it is noticed that nearly the same responses of S. Cholesterol and S. LDL-C reduction were achieved in diabetic patients when they are using doubled doses that are used for non-diabetic patients.So,higher doses of Atorvastatin (double doses) are needed to improve serum lipid levels in diabetic patients as compared to non-diabetic patients.

بما أن اضطرابات الدهون وخطر الأمراض القلبية الوعائية اكثر عند المرضى المصابين بداء السكري, وأنّ تأثيرات عقار الستاتين على مستويات الدهون في مصلِ الدم معروفة إلا ان الدراسات السابقة لَم تقارن تأثيرات عقار الستاتين على مستويات الدهون في مصلِ الدم عند المرضى المصابين بداء السكري مع غيرالمصابين, وبهدف تَحرّي تأثيرات عقار الاتورفاستاتين (Atorvastatin) على مستويات الدهون في مصلِ الدم عند المرضى المصابين بداء السكري بالمقارنة مع المرضى غير المصابين بالسكري الذين يعانون من زيادة الدهون بالدم ، اجريت هذه الدراسة على 33 مريض مصاب بداء السكري من النوع الثاني بالمقارنة مع 34 مريض غير مصاب بالسكري جميعهم لديهم زيادة مستوى الدهون بالدم ، تتراوح اعمارهم بين 40-80 سنة, قسموا الى مجاميع ليستخدموا عقار اتورفاستاتين 10, 20، و40 ملغ يومياً وليكملوا فترة متابعة مدة 6 شهور. تم قياس مستويات الدهون في مصلِ الدم قَبل وبعد المعالجة في الشهر الاول والثالث والسادس.اظهرت النتائج أن التغييرات في الكولستيرول الكلي والكولستيرول واطئ الكثافة في مصل الدم كانت أقل في المرضى المصابين بالسكري من تلك التي في المرضى غير المصابين بالسكري الذين استخدموا نفس الجُرَع من عقار الاتورفاستاتين (Atorvastatin)، بينما كانت التغييرات في الكولستيرول عالي الكثافة والدهون الثلاثية متماثل تقريباً في المجموعتين. علاوة على ذلك يُلاحظ بأنّ الاستجابة كانت نفسها تقريباً للكولوستيرول الكلي والكولستيرول واطئ الكثافة في المرضى المصابين بالسكري عندما يستخدمون جُرَع مُضاعفة كالتي استخدمت للمرضى غير المصابين بالسكري. نستنتج من ذلك أن المرضى المصابين بالسكري يحتاجون الى جُرَع أعلى (جُرَع مضاعفة) من عقار الاتورفاستاتين (Atorvastatin)، لتحسين الاستجابة العلاجية لمستويات الدهون في الدم بالمقارنة مع المرضى غير المصابين بالسكري.

Abstract

ABSTRACT: BACKGROUND: Leptin is the protein product of the ob gene, secreted by adipocytes. It has been suggested that it may plays an important role in regulating appetite and energy expenditure, but beside that, little is known about the physiological actions of leptin in humans. OBJECTIVE: To evaluate a possible influence of primary hypothyroidism on serum leptin levels. METHODS: Fifty-six newly diagnosed patients with primary hypothyroidism (40 females and 16 males) and 32 normal controls matched for age, ethnic status and body mass index (BMI) were studied. Body mass index (BMI; kg/m2), thyroid function(using enzyme-linked immunoflourescent assay) and serum levels of leptin, thyroid autoantibodies (measured by enzyme-linked immunosorbant assay) and lipid profile(measured by enzymatic colourimeteric assays) were assessed in all studied subjects. RESULTS: No significant difference in serum leptin levels was recorded between hypothyroid patients and controls (16.3±14.9; 14.8 ± 12.9, P> 0.05), but women in each group had significant higher leptin concentrations than men (patients: 19.6 ±16.3 vs. 8.3 ±5.0; controls: 19.0 ±14.4 vs. 7.7 ±4.1; P< 0.05). Serum levels of cholesterol (p<0.002), LDL-cholesterol (p<0.004) and atherogenic ratio (p<0.03) were generally higher in patients than controls. The serum leptin concentration correlated positively with BMI within both patients (r=0.32; p<0.016) and controls (r=0.28; p<0.024). However, no association was demonstrated between values of serum T3, T4, TSH, triglyceride, HDL-cholesterol and thyroid auto antibodies. CONCLUSION: Circulating thyroid hormones do not appear to play any significant effect on leptin levels in patients with primary hypothyroidism.

Abstract

Since the inflammatory factor IL-6 known by its critical role in different metabolic reactions in the body, therefore, this study was designed to detect it is effect in the thyroid gland function by injecting three groups of rats, the first with IL-6 , the second with carbimazole (antithyroid drug) and the third with normal saline (Control).The experiment included three groups of rat; each was injected with recombinant human interleukin-6 (rhIL-6), Carbimazole, or normal saline (Control). The results of experiments showed that both IL-6 and carbimazole caused a decrease in the levels of thyroid hormones (T3 and T4) in animal sera, and a significant decrease in animal body weight, but had no effect on the liver weight.There was a significant decrease in glucose level in the animals injected with carbimazole, while in the IL-6 treated animals there was no change in glucose level. There was a significant increase in serum level of triglyceride and very low density lipoprotein cholesterol in the animals treated with IL-6, while in the animals treated with carbimazole, there was no such change. Additionally, injection of the animals with IL-6 or carbimazole caused a significant increase in lymphocyte apoptosis of peripheral blood

نظراً للدور المهم الذي يؤديه العامل الالتهابي مادة بين إبيضاض-6 (IL-6) في العمليات الأيضية المختلفة في الجسم ؛ لذلك وجدنا إنه من الضروري دراسة تأثيره في وظيفة الغدة الدرقية و فعاليات أخرى و مقارنته مع تأثير عقار الكاربيمازول. وقد تم ذلك عن طريق حقن ثلاث مجاميع من الجرذان ، الأولى بمادة بين إبيضاض-6 ، و الثانية بعقار الكاربيمازول ، و الثالثة بالمحلول الملحي الفسيولوجي بوصفها سيطرة. بينت هذه التجربة إن كل من مادة بين إبيضاض-6 و الكاربيمازول يسببان إنخفاض مستويات هرمونات الدرقية (T4,T3) في مصل الدم عند حقنها في الحيوانات ، كما و يسببان إنخفاضاً معنوياً في وزن الجسم في حين لم يكن لهما تأثير في وزن الكبد. و لوحظ إنخفاض مستوى الكلوكوز في الدم لدى الحيوانات المحقونة بعقار الكاربيمازول ، بينما لم يحدث تغير في مستواه بتأثير مادة بين إبيضاض-6.أما بالنسبة لأنواع الدهون فقد لوحظ إرتفاع معنوي لمستوى كل من Triglyceride(T.G) وVery low density lipoprotein cholesterol (VLDL-C) في مصل دم الحيوانات المعاملة بـ مادة بين إبيضاض6- ، في حين لم تتأثر مستويات الدهون في الحيوانات المعاملة بالكاربيمازول. وأدى حقن الحيوانات بمادة بين إبيضاض-6 و الكاربيمازول أيضاً إلى زيادة موت الخلايا اللمفية المبرمج في الدم المحيطي .

Abstract

To investigate the effects of carbamazepine on serum glucose concentrations, lipid profile, blood pressure and prevalence of metabolic syndrome in epileptic patients. Seventy epileptic patients involved in this study. They have primary generalized and partial seizures with duration of treatment with carbamazepine of 11.5±8.37 years. Another 100 apparently healthy individuals were also participated in the study as a control group. Ten ml of venous blood samples were taken from each patient and control after 14 hours fasting. The obtained serum was separated and used for the estimation of glucose concentration, total cholesterol, triglycerides and HDL-cholesterol by using special Kits. LDL-cholesterol was determined by Friedewald equation. Metabolic syndrome was determined according to ATP III criteria for the diagnosis of metabolic syndrome. The patients and controls were matched regarding age and sex. Weight, BMI and waist circumference were statistically not different between patients and controls. Comparison between the measured variables shows significant elevations of the lipid parameters in the patient group as compared with the control group. Non significant differences obtained between serum glucose concentrations and significant low values of BP were obtained in the patient group as compared with the controls. The number of patients and control having metabolic syndrome according to ATP II diagnostic criteria for metabolic syndrome were 4 and 3, respectively. The difference is not significant. This study demonstrated that carbamazepine have no effects on serum glucose concentrations, but have elevating effects on lipid parameters and reducing effects on BP. It has no effects on the prevalence of metabolic syndrome in the epileptic patients

Abstract

ABSTRACT:BACKGROUND:Polycystic ovary syndrome is a very common condition affecting 4% to 18% of women. Polycystic ovary syndrome is associated with metabolic features and diabetes and cardiovascular disease. OBJECTIVE:To evaluate the effect of metformin on serum lipids in overweight and obese women with polycystic ovary syndrome. METHODS:An observational prospective study conducted at AL- Diwanyia Maternity and child Hospital from May 2011 to March 2013. All women in this study have BMI >27 (range) so, they are all overweight and obese .The women randomized into two group; one group were recieved metformin therapy for 4 months duration and the other group were kept on no therapy and followed for the same period. All studied women were subjected to measurement of fasting Blood sugar, and measurement of overnight fasting lipid profile which include total cholesterol, triglycerides, high density lipoprotein cholesterol and low density lipoprotein cholesterol.RESULTS:For women who received metformin, there was significant increase in the level of high density lipoprotein P = 0.04. The value of triglyceride although decreased after metformin treatment, the difference was not significant P > 0.05.Other parameters (cholesterol, low density lipoprotein cholesterol) showed no difference. Fasting blood sugar was not changed during the study. CONCLUSION: Metformin have been shown to improve the lipid profiles leading to increase the high density lipoprotein cholesterol, the main predictive of cardiovascular disease in the women with polycystic ovary syndrome.

Abstract

Background: Benign Prostate Hypertrophy(BPH)is a common urological problem worldwide which is defined as denomatous hyperplasia of the periurethralpart of prostate gland that occurs especially in men over 50 years old and that tend to obstruct urination by constriction the urethra Objectives: The study was aimed to investigate the level of Malondialdehyde (MDA), Nitric Oxide (NO) and Superoxide Dismutase (SOD) as an antioxidant, besides other factors such as the level of Lipids Profile (Total Chlosterol (TC), High Density Lipoprotein Cholesterol (HDL-C), Low Density Lipoprotein Cholesterol (LDL-C), Very Low Density Lipoprotein Cholesterol (VLDL-C), and Triglyceride (TG))in patients suffer from BPH . Methods: In this study ;clinical ,specific prostate antigen and sonongraphy data of 80 persons were prospectively analyzed ; they are divided into two groups (40) male patients with Benign Prostate Hypertrophy (BPH) was previously diagnosed and another (40) healthy males as a control group .The study was performed at Al-Khadhmyah teaching hospital during the period from March 2001 till April 2003Results: The evaluated data recorded a significant increase in the levels of (MDA) and (NO), while a significant decrease in the level of (SOD) (P  0.001) in patients complain from BPH in comparison with the control group. Moreover, the results showed a significant increase in the levels of (TC), (TG) , (LDL-C) and a significant decrease in the level of (HDL-C) (P  0.001) in comparison with the control groupConclusions: Benign Prostate Hypertrophy (BPH) is the most common benign tumor in men and Malondialdehyde (MDA), Nitric Oxide (NO) ,Superoxide Dismutase and Lipid profile are beneficial markers to evaluate patients with BPH in comparison with those without BPH.

Abstract

Due to the misuse of fats and oils during deep fat frying for preparing many types of foodlike (Falafel) and its effect on the health and economic aspects and to optimize the use offats and oils . This work includes the chemical and physical changes of local common oil(Al – Bint) and imported oil and fat (CEZER, Two Golden Spoons) before and duringdeep frying operating at 5±180°C, it also includes sensory evaluation of Falafel duringfrying operation. The results of this research were summarized as follows:-1- Significant decrease of Iodine Number for the three fats with increasing frying times.The decrease was from (138.8, 137.9, 54.5) before process of frying to (131.5, 133.9,49.2) after the fourth frying time for Al – Bint, CEZER oils and Two Golden Spoons fatrespectively. Consequently the decrease of Iodine Number were (4.3, 4.0, and 5.3) for thethree fats respectively.2- Significant decrease of Smoke Point for the three fats with increasing number offrying times. The decrease of Smoke Point was from (213°C, 200°C, 214°C) beforefrying process to (205°C, 190°C, 206°C) after the fourth frying time for Al – Bint,CEZER oil and Two Golden Spoons fat respectively. Consequently the resistance todecrease of Smoke Point of Two Golden Spoons fat more than Al – Bint oil then CEZERoil.3- Significant increase in Refractive Index at (40°C) with increasing number of fryingtimes for the three fats. The increase was from (1.4473, 1.4474, and 1.4470) beforefrying process to (1.4710, 1.4740, and 1.4640) after the fourth frying time for Al – Bint,CEZER oils and Two Golden Spoons fat respectively. Consequently the stability foroxidation for Two Golden Spoons fat more than Al – Bint oil then CEZER oil.1- Sensory evaluation of Falafel during deep frying for several times appeared that theappearance, texture, color, flavor and overall acceptance were acceptable. Falafel fried inAl – Bint oil was the best, then that with CEZER oil and then with Two Golden Spoonsfat. Also there were no significant effect of number of frying times on texture, flavor,color and overall acceptance of Falafel.

Abstract

The estimation of oocyte quality in human in vitro fertilization (IVF) is an important point for the embryologists. Oocyte selection and the identification of the best oocytes may help to limit an overproduction of embryos and to improve the results of oocyte cryopreservation programs.The follicular fluid (FF) can be provided easily during oocyte pick-up and known to represents an optimal source for non-invasive biochemical predictors of oocyte quality. However, till now no substance was found to be used as reliable markers of oocyte competence to fertilization, embryo development and pregnancy.Metabolism and ATP levels within the oocyte and adjacent cumulus cells are associated with quality of oocyte and optimal development of a healthy embryo. Lipid metabolism provides a potent source of energy and its importance during oocyte maturation is being increasingly recognised.

Abstract

ABSTRACT:BACKGROUND:To evaluate serum lipids in children with type 1 diabetes mellitus and its relation to glycemic control, in comparison with sex and age matched non-diabetic children.METHODS:Fasting blood glucose, glycosylated haemoglobin (HbA1c) and lipid profile were estimated in 45 children (20 males and 25 females) with type 1 diabetes attending diabetic clinic at Sulaymani children hospital from the period of November 2005 till Jully 2006. Fifty health children (23 males and 27 females) admitted to the same hospital with acute illnesses like respiratory tract infections were randomly selected as controls.RESULTS:Mean total cholesterol, triglycerides, LDL-C, VLDL-C, and LDL-C/HDL-C ratio were significantly higher in diabetic children compared to the control. However, there was no significant difference in the levels of HDL-C in both groups .In addition to that, diabetic patients with poor control have a significantly higher levels of blood glucose, total cholesterol, triglycerides, LDL-C and LDL-C/HDL-C ratio as compared to those with good control. A significant correlation between most lipid variables and HbA1c was observed in diabetic patients.CONCLUSION:Children with type 1 diabetes should be screened for serum lipids as significant lipid abnormalities are related to glycemic control. Both can be improved with dietary guidelines and insulin therapy.
