Abstract

Background: Rheumatoid Arthritis (RA) is heterogenous syndrome. Because the diversity of diseaseprocesses and formation of complex lymphoid microstructures that indicate the multiple T cell activationpathways are involved .affected patients have major abnormalities in the T cell pool with clonallyexpanded CD4+ T cell that lose expression of the CD28null molecule and lack the ability for profiliration.Adenosine deaminase (ADA) is an indicator of the proliferation and differenation of lymphocyte, inparticularly the T cell subcells.Patients and Methods: Total ADA levels were measured in the sera of RA patients and healthy groupaccording to Giusti (1981).Results: The mean value of ADA was lower in patients with RA than control group with no significantdifferences.Conclusion: the lower value of ADA (which involved in the proliferation of lymphocyte) in RA patientsmay results from the predominance of CD4+ T cells in the peripheral blood

Abstract

Background:Rheumatoid arthritis (RA) is chronic systemic inflammatory disease. Porphyromonus gingivalis (P. gingivalis) produce peptidyle arginine enzymes, which lead to citrullination of human protein then lead to formation anticitrullinated peptide antibody (ACPA).Objective:To investigate the role of P. gingivalis as environmental factor for RA and association of P. gingivalis with development of ACPA.Methods:This study included 31 newly diagnosed RA patients with periodontitis, which included 22 females and 9 men in addition to 30 individual as healthy controls, which included 20 females and 10 men. The exclusion criteria included autoimmune disease (systemic lupus erythematosus, Bachet disease, ankylosing spondylitis, multiple sclerosis), attending Department of Rheumatology in Baghdad Teaching Hospital during period from May 2014 to January 2015. The age range was 20 to 68 years. Disease activity score 28 (DAS28) was calculated for each patient. Five ml of blood sample was taken for detection of ACPA antibody while gingival cervicular fluid was taken by paper point for detection of P. gingivalis by polymerase chain reaction (PCR) with specific primer for fimbrial antigen (fimA).Results:The frequency of positive cases with P. gingivalis were 13/31(41.90%) while in healthy controls was zero with significant P value (<0.001). The association between anti cyclic citrullinated peptide (ACCP) antibody and frequency of positive cases for P. gingivalis was significant (P<0.041). The association of P. gingivalis positivity and DAS28 was non-significant (P= 0.003).Conclusions:P. gingivalis showed positive association with RA in newly diagnosed patients. The frequency of positive cases for P. gingivalis revealed association with positivity of anti-CCP.Key words:RA, ACCP, P. gingivalis

Abstract

Rheumatoid arthritis is an inflammatory, autoimmune disease that causes pain, joint stiffness and loss of function. It can occur at any age but is more common in persons over the age of 30 years and affects women more often than menThe Aims of the study is Compare changes in Adenosine deaminase activity among patients with rheumatoid arthritis and control group , Compare total anti-oxidant for patients with RA and control group The study was conducted in rheumatology unit in Marjan teaching hospital in Hilla city / Babil / Iraq from 1st December 2014 to 1st February 2015.The present study was conducted on (60) patients with rheumatoid arthritis and (53) apparently healthy subjects were taken as control group and the ages between (30–79) years. The sera obtained from the blood of patients and healthy subjects were used to measure the concentrations of Adenosine deaminase and Total antioxidant capacity ADA is an important enzyme of purine metabolism catalyzing the irreversible deamination of adenosine to form inosine and is considered as a marker of cell mediated immunity. RA patients have lower levels of serum antioxidants comparison to healthy person.In comparison with the control group, the patients with RA showed a significant increase in of Adenosine deaminase (P ≤ 0.05) concentrations (120.91 ng/ml ± 7.171 vs. 50.717 ng/mL ± 5.613), and a significant decrease in Total antioxidant capacity (P ≤ 0.05) concentration(2.8214 ± 1.118 vs. 9.413 ± 1.48).

Abstract

Background: Rheumatiod factor in patients with Rheumatoid arthritis (RA) could be of IgM, IgG, or IgG isotypes.
Aim: To study the occurance of each or all these isotypes in RA patients .
Methods: Enzyme linked Immune Sorbent Assay (ELISA) & Latex agglutination test were used to asses RF in 74 patients with RA.
Results: Using ELISA, 58 (78.4%) patients had positive RF (IgM 48.6%, IgG in 47.3% & IgA in 54.1% ), while only 43 (58.1) patients were positive by AG .
Conclusion All RF isotypes should be assessed in patients with RA

Performance of Anticyclic Citrullinated Peptide Antibodies versus Rheumatoid Factor in diagnosis of Rheumatoid Arthritis

Abstract

The aim of this study was to evaluate the performance of anti-cyclic citrullinated peptide antibodies (anti-ccp) versus Rheumatoid Factor (RF) in the diagnosis of rheumatoid arthritis in serum sample of patients with clinical manifestation of arthritis using ELISA technique.One hundred and twenty patients with clinical presentation of rheumatoid arthritis and fifty apparently healthy individuals (25 males and 25 females) were enrolled in this study. The mean age of patients group and control was 36.45years versus 34.72 years .Among patients, males represented (8.3%) compared with (91.7%) of females. In control group males represented (52%) versus (48%) females.There was a statistical significant difference (p>0.05) between patients and control group in RF and anti-ccp values. Eighty five percent of patients gave positive results for anti-ccp compared with (100%) negative results in control group. RF was detected in (41.66 %) of patients sera compared with (58.33 %) which gave negative results. RF ELISA gave negative results in (100%) of control group .Only 10(8, 33%) out of 120 patients gave negative results in RF and anti-ccp ELISA, compared with 42(35%) out of 120 gave positive results in both tests. In 60(50%) out of 120 RA patients RF gave negative results and at the same time gave positive results when retested using anti-ccp ELISA .Only 8(6.66%) out of 120 RA patients gave negative result in anti-ccp ELISA technique and RF was detected in positive value.The sensitivity of anti-ccp ELISA was (85%) versus (41.66%) for RF. The Specificity of anti-ccp ELISA was (55.55%) versus (14.28%) for RF. Positive predictive value for anti-ccp ELISA was 41.17% versus (84%) for RF. Negative predictive value for anti-ccp ELISA was 55.55% versus 14.28 for RF ELISA .False positive value in anti-ccp ELISA was 58.82% versus 16%for RF ELISA. False negative value in anti-ccp ELISA was 44.44% versus 85.71%for RF .This study concludes that anti-ccp ELISA was more sensitive and specific in diagnosis of RA than RF ELISA technique.

Abstract

This study involved (50) patient’s that complaining rheumatoid arthritis from both genders (7 females and 43 males). Their ages range from (26-68) years there are taken from hospitals and clinics, in addition to the control which involve (30) person that look normal and their ages ranges from (23-54).Enzyme linked immuno-sorbent assay (ELISA) has been used to measure the level of IL-6 in serum, in which the medium of IL-6 in patients was significantly higher (p ≤ 0.01) than control group.The study show’s significant difference (p ≤ 0.01) in level of IL-6 between males and females and between age groups.

شملت هذه الدراسة (50) مريضاً بالالتهاب المفاصل الرثوي من كلا الجنسين (7 إناث و43 ذكور) تراوحت أعمارهم بين (26-68) سنة من ضمن الحالات الواردة إلى المستشفيات والعيادات الخارجية، فضلاً عن مجموعة السيطرة التي شملت (30) شخصاً من الأصحاء ظاهرياً وقد تراوحت أعمارهم بين (23-54).تم استخدام اختبار الامتزاز المناعي المرتبط بالانزيم Enzyme Immuno-sorbent assay (ELISA) لقياس تركيز IL-6 في المصل وقد كانت قيمة المتوسط الحسابي للـ IL-6 لدى المرضى أعلى وذات فرق احصائي عالي المعنوية (P ≤ 0.01) عنها في مجموعة السيطرة.وقد أظهرت الدراسة اختلاف عالي المعنوية (P ≤ 0.01) في مستوى IL-6 بين الذكور والإناث وكذلك بين المجاميع العمرية.

Abstract

Electrochemical machining is one of the widely used non-conventional machining processes to machine complex and difficult shapes for electrically conducting materials, such as super alloys, Ti-alloys, alloy steel, tool steel and stainless steel. Use of optimal ECM process conditions can significantly reduce the ECM operating, tooling, and maintenance cost and can produce components with higher accuracy. This paper studies the effect of process parameters on surface roughness (Ra) and material removal rate (MRR), and the optimization of process conditions in ECM. Experiments were conducted based on Taguchi’s L9 orthogonal array (OA) with three process parameters viz. current, electrolyte concentration, and inter-electrode gap. Signal-to-noise (S/N), the analysis of variance (ANOVA) was employed to find the optimal levels and to analyze the effect of electrochemical machining parameters on Ra and MRR. The surface roughness of the workpiece was decreased with the increase in current values and electrolyte concentration while causing an increase in material removal rate. The ability of the independent values to predict the dependent values (R2) were 87.5% and 96.3% for mean surface roughness and material removal rate, respectively.

تعتبر عملية التشغيل الكهروكيميائي واحدة من عمليات التشغيل اللاتقليدي لتشغيل الاشكال المعقدة للمواد ذات التوصيل الكهربائي صعبة التشغيل كسبائك النحاس , سبائك التيتانيوم, سبائك الفولاذ,الفولاذ المقاوم للصدأ, الخ . ان استخدام ظروف التشغيل المثالية تعمل على تقليل وقت التشغيل للماكنة وكذلك كلفة الصيانة وسوف تعمل على انتاج اجزاء ذات دقة عالية. في هذا البحث تم دراسة تأثير ظروف التشغيل على الخشونة السطحية ومعدل ازالة المعدن والظروف المثالية للعملية. انجزت الاختبارات بالاعتماد على مصفوفة تاكوجي العمودية مع ثلاثة ظروف تشغيل وهي التيار وتركيز المحلول ومقدار الفجوة بين الاداة والمشغولة. استخدمت نسبة الاشارة الى الضوضاء وتحليل التباين لايجاد المستويات الامثل وتحليل تأثير ظروف التشغيل على الخشونة السطحية وعلى معدل ازالة المادة. تقل الخشونة السطحية للمشغولة بزيادة قيم كل من التيار وتركيز المحلول في حين انه يسبب زيادة في معدل ازلة المادة. قابلية القيم المستقلة على التنبؤ بالقيم المعتمدة هي 87.5% و 96.3% للخشونة السطحية ومعدل ازالة المادة على التوالي

Abstract

Background: The tumor necrotic factor alpha (TNF-α) was associated with rheumatoid arthritis (RA) pathogenesis and inflammation of joint, for this consider important target for drugs (TNF inhibitors) as for example infliximab.Objective: To investigate the role of TNF-α-308 polymorphism G>A in unresponsiveness to biological treatment (infliximab) in RA patients.Methods: This study included 29 RA patients treated with infliximab attending Department of Rheumatology in Baghdad Teaching Hospital and diagnosed according to American College of Rheumatology (ACR) with 30 persons as healthy controls. The age range was 20 to 68 years. The DAS28 was calculated for each RA patients. Blood samples were taken from them during the period from May 2014 to January 2015. The blood samples were tested by polymerase chain reaction (PCR) for TNF-α-308 G>A then by restriction fragment length polymorphism (RFLP) for A allele and G allele. HLA-DR genotyping by PCR-SSO.Results: The frequency of A allele was 23 (39.7%) in RA patients while G allele was 35 (60.3%) in RA patients, whereas in in controls A and G alleles frequencies were 5 (8.3%) and 55 (91.7%) respectively. The A allele was associated with high DAS28 19 (82.6% while for G allele was 1 (2.9%) and DAS28 showing significant association with TNF-α-308 polymorphism G>A. The distribution of A allele in female RA patients was 16 (36%) whereas G allele was 28(64%), while in male RA patients, A and G allele were 7 (50%) for each of them. The HLA-DRB1 not showing significant association with TNF-α-308 polymorphism G>A.Conclusion: The TNF-α-308 polymorphism G>A has effect on response to biological treatment (infliximab). The A allele of TNF-α was associated with unresponsiveness to infliximab and RA patients who carry this allele have high DAS28 score. Abstract

Non-traditional machining process is more used to manufacture geometrically complicated and an accurate parts for electronics, aerospace and automotive industries. Chemical machining process is one of non-traditional machining methods, it is as well named as chemical etching. The current research is aimed to study the influence of the machining time, machining temperature, etching solution concentration on the material removal rate and surface roughness of aluminum alloy by using mix of acid FeCl3. There are three of machining temperatures (25, 30 and 35 ºC) with three machining times (4, 8, and 12min) and etching solution concentration (25%, 50%, and 75%) were used as machining conditions. These conditions are significant variables that have effect on finishing performance of chemically machined aluminum alloy. Machining time has the greatest effect among these variables. The time is the most important parameter for maximum Material Removal Rate (MRR), and the interaction between temperature and etchant concentration is the next important parameter for maximum MRR. The time is the greatest parameter for minimum Ra, the interaction between time and temperature is the next significant parameter for less Ra.
