E ffect of Ventricular Septal Defect on the Growth Pattern of Children

Abstract

ABSTRACT:BACKGROUND:VSD is the most common cardiac malformation accounting for 25% of congenital heart disease. The VSDs are classified according to its haemodynamic effect into small, moderate and large. The VSD can affect the growth, which is defined as a progressive increase in size of the body as a whole or of its separate parts and can result in failure to thrive in affected patients.AIM OF THE STUDY:This study was done to demonstrate the effects of the VSD on weight, height and occiptofrontal circumference (OFC) of children as parameters of growth.METHODS:This is a prospective study of 50 patients with isolated VSD done in welfare Teaching hospital and Ibn Al-Bitar cardiac center during the period from January to July 2005. The diagnosis of VSD was done by clinical picture and confirmed by echocardiographic examination to study the effect of malnutrition and other factors on the growth another 50 patients without VSD were included as control group. For each patient the growth parameters (weight, height and head circumference) were measured.RESULTS:The age of the patients with VSD ranged from (14 days to 14 years). The male to female ratio was 1.1 to 1. In patients with VSD the weight of 20 patients (40%) were below 3rd centile in comparison with 3 patients in the control group and this is statistically significant (P<0.001), while the differences in the height of both groups was not statistically significant. The head circumference of 14 patients (28%) were below 2%, while, 3 patients (6%) only in control group and this difference was statistically significant (P<0.005). The effect of VSD on the growth parameters depended on the size of the VSD. There was a significant effect of moderate and large VSD on both weight and head circumference while small VSD had no such effect.CONCLUSION:From this study, we conclude that the weight is the most sensitive parameter for studying the effect of VSD on the growth parameters. The patients with moderate VSD, should be managed as serious as large VSD because of its adverse effect on growth.

Abstract

Back ground: Ischemic VSD or ventricular septal rupture (VSR) is a rare but lethal complication of myocardial infarction (MI). Multi-system coronary disease is present in more than 50% of these patients. The event occurs 2-8 days after an infarction and often precipitates cardiogenic shock. The differential diagnosis of postinfarction cardiogenic shock should exclude free ventricular wall rupture and rupture of the papillary muscles. To avoid the high morbidity and mortality associated with the disorder, patients should undergo emergent surgery. Concomitant coronary artery bypass may be required.Objective: This study stressed on a very serious complication of a common pathology (myocardial infarction), despite it's lethal complication but still it is treatable and still surgery is the only salvageable option. Patients & methods: Retrospective study of 7 consecutive cases with ischemic VSR encountered from (1st Jan. 2008-30 of September 2011). All of the cases were carefully assessed & fully investigated preoperatively (by echocardiography study, catheterization ...etc) & for all of them emergency open heart surgery was done with heart lung machine to close a post infract VSD using synthetic patch and ventriculotomy closed with reinforced Teflon felts , all of them(except the last case which died immediately post operatively) stayed for 2-3 days in the CICU under follow up .Results: Seven patients have been operated upon, 4 males (57%) and 3 female (43%), and their ages ranged between 50-63 years (75% in the 6th decade). Three of them (43%) were diabetic only but all of them were hypertensive. All of them were haemodynamically unstable so operation carried on emergently within 12-18 days post infarction (one operation done within 27 hours of infarction) and in none of them the clinical status helped to postpone them until more than 3 weeks post infarct. Five Out of the seven patients survived the operation, one died immediately post operatively &another one died 7 days post operatively due to renal failure (due to pre operative impaired renal function). Operative & early post operative mortality is (28%).Conclusion: We need to be more aware about this fatal complication of acute MI taking in consideration its late presentation (2-8) days.We have to work hardly to improve early revascularization to decrease the incidence of VSR. Improvement in surgical technique using double patch and glue improve mortality & morbidity rates. المقدمه:- أن ثقب جدار القلب بين البطينين الناتج عن أحشاء العضلة القلبية وبالرغم من ندرته ألا أن مضاعفاته مميتة.وتحدث هذه المضاعفات عادة بعد مرور حوالي 2_8 أيام من أحتشاء العضلة القلبية وعادة ماتنتهي بهبوط حاد بالدورة الدموية ولغرض تفادي المضاعفات وحالات الوفاة العالية والمصاحبة لهذه الحاله. يجب أجراء العملية الجراحية بصوره طارئة وعادة يحتاج المرضى بالاضافه الى تصليح جدار القلب لزراعة شرايين القلب التاجيةالغرض من البحث:- أن البحث يركز على مضاعفات خطره لحاله مرضيه شائعة (أحتشاء العضلة القلبية) وبالرغم من كونها مميتة ألا أن ألجراحه تعتبر الوسيلة الوحيدة فقط لإنقاذ حياة المريض.الطرق والمواد:- أن البحث أشتمل على دراسة لحالات (7) مرضى يعانون من ثقب في جدار البطينين في القلب نتيجة أحتشاء العضلة القلبية لفترة من (الأول من كانون الثاني 2008 إلى الثلاثين من أيلول 2011).جميع الحالات تم أدخالهم الى المستشفى وأجريت لهم كافة الفحوصات اللازمه لتثبيت تشخيص الحاله.جميع المرضى تم أجراء عمليات جراحيه طارئه لهم (عملية قلب مفتوح لغلق الفتحة بين البطينين مع زرع الشرايين التاجيه وبأستجدام ماكنة القلب والرئه الصناعيه ) جميع المرضى تمت متابعتهم بعد العمليه داخل العنايه القلبيه الجراحيه المركزه لفترة 2_3 أيام (بأستثناء حالة واحده حيث توفيت مباشرة )النتائج:- بعد أجراء عمليات جراحيه لسبعة مرضى كان 4 منهم رجال و 3 منهم نساء وتراوحت أعمارهم بين 50_63 سنه .ثلاثة مرضى كانو يعانون من داء السكري وجميعهم يعانون من أرتفاع ضغط الدم جميع المرضى كانت حالاتهم غير مستقره ويعانون هبوط في الدوره الدمويه وتم أجراء العمليات الجراحيه لهم بعد مرور (12_18 يوم ) من أصابتهم بأحتشاء العضله القلبيه ..خمسة مرضى تماثلو للشفاء بعد أجراء العمليه الجراحيه وأثنان من مجموع 7 مرضى توفيا أحدهما نتيجة عجز كلوي بسبب هبوط حاد للدوره الدمويه قبل العمليه والثاني توفي بسبب تلف كبير بعضلة القلب.الاستنتاج:- تحتاج هذه الحالات الى أدراك سريري عالي لتشخيصها كونها تحدث بعد عدة ايام من دخول المرضى الى ردهة العنايه القلبيه الفائقه وبالرغم من كونها حالات مميته ألا أن الجراحه تكون منقذه للحياة أذا تم أجراؤها في الوقت المناسب قبل تدهور الحاله السريريه للمرضى .المفاتيح و الكلمات:-•ثقب جدار البطينين في القلب .•أحتشاء العضله القلبيه.

Abstract

ABSTRACT:BACKGROUND :Coronary artery bypass grafting (CABG) can be divided according to it's timing into:elective, urgent, emergency and salvage.The purpose of urgent CABG is to reperfuse the coronary artery blood flow rapidly, improves areas of myocardial ischemia, repair ventricular dysfunction and prevent the spread of the infarction area. Urgent CABG has a higher morbidity and mortality than elective CABG.OBJECTIVE:The purpose of this study was to identify preoperative and peroperative risk factors associated with this morbidity and mortality, evaluate clinical outcomes and recommend the possible solutions.MATERIALS AND METHODS:This is a retrospective study that reviewed the hospital records of 50 patients who underwent urgent CABG at the Iraqi Center for Heart Diseases for three years (2008-2010). The main daignostic tools were coronary angiography and echocardiography. All underwent conventinal CABG on cardio-pulmonary bypass with cardioplegic arrest. Variables that may be related to operative risk were analaysed.RESULTS :Forty-three patients underwent isolated CABG, while six patients underwent postinfarction ventricular septal defect repair and one patient underwent mitral valve replacement in addition to CABG, Postoperative mortality was 20%.CONCLUSION:The most common cause of mortality was Low Cardiac Output Syndrome followed by Acute Renal Failure.Preoperative risk factors for perioperative mortality were age more than 65 years, preoperative cardiogenic shock and low ejection fraction below 40.KEYWORDS: urgent CABG, postinfarction VSD, low cardiac output syndrome.

Abstract

ABSTRACT:BACKGROUND:Conventionally,VentricularSeptal Defects(VSDs) are repaired with synthetic patch-Dacron(polyetheylene terephthalate) or Goretex(expanded polytetraflouroethylene).Recently,we began using glutaraldehyde-treated autologous pericardial patch to repair VSDs.We review our experience.OBJECTIVE: In this study,we are evaluating the result of closure of VSD by pericardial patch instead of synthetic patch.MATERIALS AND METHOD:BetweenGanuary 2009 to April 2014,60 children had their VSDs repairewith glutaraldehyde-treated autologous percardium.There were 20 males and 40 females, aged between 2 years and 12 years with amedian age of 4 years.The diagnosis was isolated VSD in 49 patients,multiple VSD in 3:Tetralogy of Fallot(TOF) in 8 patients The chest was opened by median sternotomyincision.After establishing cardiopulmonary bypass,a strip of pericardium was harvasted from patient and fixed in 0.6% glutaraldehyde for about 25 minutes.It was then washed out with 0.9% saline solution.The defect was repaired with 4/0 suture using a continous suture technique.RESULTS:There was no hospital mortality. Postoperative echocardiogram reveald trivial leaking in 10 patients.Follow up was for 3 to 6 months(mean 2 months). No patient required reoperation for residual VSD.CONCLUSION:Glutaraldehyd-treated autologous pericardium is an exellent material for surgical patch closure of VSD.It is easily available and does not require sterilization.

Abstract

In digital signal processing (DSP), FIR digital filter isvery important device to deal with particular frequencies of acertain signal to be appropriate for some applications such ascommunications, sound equalizers, etc. In this paper, FIRfilters are adapted to decompose the original sound signal intofour signals. Each one is created by one FIR filter and eachfilter represents a narrow band of frequencies. The filteroutput is used to drive a certain variable speed drive (VSD) tocontrol the speed of a water pump and light intensity of acolored lamp. This filter output signal is applied to the analogcontrol voltage terminals of the VSD unit to control thefrequency and magnitude of the voltage supplied to the lampand pump. Thus, the heads of the water jets and the lightintensity is controlled according to the analog control signalswhich are created by the FIR filters (The VSD is used to mapthe filter output into light intensity and water head bycontrolling the supplied voltage of them). The goal of this studyis to design and simulate four sound harmonics bandsproduced by FIR filters to drive four VSDs which aresimulated using V/F ratio constant method for musicalfountain operation.
