Abstract

objective: To evaluate the results of closed antegrade static femoral nailing for closed Winquist type III and IV fractures of shaft femur.Methodology: A descriptive study conducted at department of Orthopedic surgery in Al Kadhimya hospital from March 2008 to July 2010. Twenty eight patients are included, twenty male patients and eight females , three were missed during follow –up period, admitted to the emergency department and operated on with closed reamed interlocking ante grade nailing under general or spinal anesthesia within 72 hours. Followed up for one year looking for union rate, malunion, infection, knee joint stiffness and trendelinburg gait.Results: Union was achieved within twenty weeks in (twenty two 88%) patients, delayed union until thirty six weeks in (two 8%) patients and (one 4%) patient developed non union proved clinically and radiologically.Infection developed in two patients (8%) at the site of distal screw insertion, both were superficial and resolved conservatively.Knee joint stiffness developed in six patients (24%), from which five resolved completely within six weeks of physiotherapy and one with long term less than twenty degree knee joint stiffness.Trendelenburg gait developed in four (16%) patients, three (12%) resolved completely within ten weeks and one patient (4%) continues with Trendelenburg gait after one year.The mean operative time was 90 minutes.Malunion as shortening less than one cm developed in two cases (8%) and angulations less than 10 degrees in one case (4%).Key words: Closed, reamed, ante grade, static interlocking nails, femoral shaft fracture.

Abstract

Background: Fractures of the shaft of femur are a major cause of morbidity and mortality in patients with lower extremity injuries. Most fractures are sustained in young adults during high velocity injuries. The technique of intramedullary locked nail(IMLN) of bone wasdeveloped to overcome the rotational and longitudinal malalignments of long bone fractures.Aim of study:To evaluate the results of femoral shaft fractures treated by closed reamed antegrade interlocking nail.Patients and Method: prospective study conducted at Department of Orthopedic surgery in Al-Kadhemiyah Teaching Hospital from October 2008 to October 2010 to evaluate 20 patients with acute femoral shaft fractures, all treated with reamed antegrade IMLN inserted with closed techniques. All patients treated for established non-unions and infections or with pathological fractures were excluded from the study.Results:out of 20 patients 16(80%) were male and 4(20%)were female. The age range was from 18 to 52 years (average age of 35 years).15 patients with closed fractures (75 %) and 5 patients with open fractures (25%). The union rate was 90% with 10% of fractures going into delayed union treated with dynamizationDuration of union range from 12 to 24 weeks. At the final clinical follow-up, 2cases (10%) had minor thigh pain, 1case (5%) had limb length discrepancy of less than 2cm and 1case(5%) had decreased range of motion at the knee joint, We had 2 cases(10%) of infection. There were 1(5%) case of pudendal nerve neuropraxia .Conclusion:closed Intramedullary Interlocking nailing is a safe and effective treatment modality for acute fractures of the femoral shaft. Patient rehabilitation is early, hospitalization is short, and fracture healing response is good.

Abstract

Background:Ipsilateral concomitant fractures of the femoral neck and femoral shaft are unusual (incidence 5-6%), and pose a difficult treatment problem. This injury usually resulting from high energy trauma, road traffic accident and fall from height. Many treatment methods have been invented and adopted, controversy exists about the optimal methods of treatment for such fracture.Objectives:To assess the short-term results of ipsilateral fracture neck and shaft femur treated by modified traditional ante-grade intra-medullary nailing.Methods:Fourteen patients with ipsilateral concomitant fracture neck and shaft femur treated by modified traditional ante-grade interlocking nailing. The surgery includes fixation of both neck and shaft fracture by interlocking nail (traditional type) and multiple long lag screws inserted via the nail itself toward the head and anterior or posterior to nail. Those patients followed for maximum 48 weeks to assess the risk of infection, knee stiffness, delayed union, mal-union, and non-union. Results:One patient had superficial infection treated by oral antibiotics and improved (7.1%). Two patients had knee stiffness of mild degree (the range is 0-120) improved by physiotherapy (14.3%). Two patients had Trendelenburg’s gait (14.3%). One patient had non-union of femoral shaft (14.3%). One patient had mal-alignment of femoral neck (7.1%). One patient had mal-alignment of femoral shaft (7.1%). One patient had delayed union (failure to progress by 6 month as judged by serial radiographs) of femoral shaft (7.1%). Two patients need open reduction because of soft tissue interposed (14.3%). We lost follow up of 2 patients (14.3%).Conclusion:Modified method for fixation of ipsilateral neck and shaft fracture with traditional interlocking nails and multiple long lag screws still can be adopted especially in the absence of cephalo-medullary nails with good results regarding early results. Still the cephalo-medullary nails may be better and required other studies.Key words:Interlocking nailing, ipsilateral fracture neck and shaft femur.

Abstract

The choice of aggregate type in producingreinforced concrete depends on the availability of the sourcesometimes and the intended concrete requirements likelightweight or normal aggregate concrete or high strengthconcrete. The punching shear resistance is being considered tobe influenced by numbers of parameters including aggregatesize and types. These parameters have not accounted in most ofcodes of design and have given a little attention by researchers.Most of the available knowledge is based on outcomes fromexperimental works on beams. In this paper, the considerableslab tests without shear reinforcement are collected fromliterature in which aggregate types and sizes are given and theywere failed in punching. The test results are compared to thosecalculated by ACI, EC2 and CSCT. The deficits of shearresistance are found clear where high compressive strength iscombined with reinforcement ratio.

Abstract

It is well known that crime is a reflection of the society and one of its aspects. The crime reflect all the Circumstances in the communities, and it return to the human behavior Interlocking relations of good and evil and its continuing struggle. A new kind of criminals had entered the crime field beside the traditional criminal and those new criminals are different from traditional criminals in the style of crime Implementation, which we touched upon in this research. The research divided into three sections: the first section - the information systems and its legal nature.Second section - the legislative development of criminalization the attacks on information systems. The third section - the concept of penetrate information systems crime and its bases.

من المعلوم أن الجريمة هي إفراز للمجتمع ومظهر من مظاهره ومن ثم تعكس كافة ما تموج به المجتمعات من ظروف والتي ترجع إلى ما يحويه السلوك الإنساني في علاقاته المتشابكة من خير وشر وتصارعهما بصفة دائمة , لذا فقد اقتحم الجريمة والنشاط المعادي للمجتمع نوع جديد من المجرمين إلى جانب المجرم التقليدي يختلفون في أشخاص مرتكبي الجريمة أو في أسلوب ارتكابها وهو ما تناولناه في هذا البحث في ثلاثة مباحث الأول- لماهية النظم المعلوماتية وطبيعتها القانونية – والثاني للتطور التشريعي لتجريم الاعتداء على أنظمة المعلومات – والثالث لمفهوم جريمة اختراق أنظمة المعلومات وأركانها.
